Assumption of Risk

I understand that participation in dance, musical theatre, acting, voice, fitness, acrobatics, rehearsals, performances, competitions, camps, workshops, and all other activities offered by Taylor Made Dance & Theatre (“TMDT”) involves physical activity and carries inherent risks, including but not limited to:

* Falls, slips, trips, and collisions.
* Muscle strains, sprains, fractures, dislocations, and other injuries.
* Physical exertion that may result in illness or injury.
* Injuries caused by props, costumes, scenery, equipment, or other participants.
* Risks associated with performances, travel, and off-site events.

I voluntarily choose to participate and fully accept all known and unknown risks associated with participation.
To the fullest extent permitted by law, I hereby release and hold harmless Taylor Made Dance & Theatre, its owners, directors, instructors, choreographers, staff members, volunteers, assistants, independent contractors, and facility owners from any and all claims, demands, actions, damages, losses, or liabilities arising out of or related to participation in any TMDT activity, except in cases of gross negligence or willful misconduct where prohibited by law.

This release includes claims arising from personal injury, illness, property damage, or other losses.

I grant permission for Taylor Made Dance & Theatre to photograph and/or record myself or my child during classes, rehearsals, performances, competitions, camps, workshops, and events.

These images and recordings may be used without compensation for promotional purposes, including:

* Social media
* Website
* Advertising
* Brochures
* Newsletters
* Print materials
* Video promotions
