I have received or will obtain a copy of Oregon Ballet Academy policies and will take the responsibility to carefully read and follow the rules and policies therein. I understand that Oregon Ballet Academy does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. There are no partial or full tuition refunds except in the case of physical impairment, requiring a doctor’s note.
I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child’s participation with Oregon Ballet Academy, and I voluntarily assume the risks associated with such participation. I do hereby waive, release and forever discharge OBA and OBF and its employees, instructors, and staff from any and all responsibilities or liability for injuries or damages resulting from my or my child's participation in any activities. Furthermore, I agree to indemnify each of them from any loss, claim, damage, or expenses, including attorneys' fees resulting from or arising out of any injury to any person or damage to property, caused by participation of Releaser in any activities at OBA and OBF.
I further authorize OBA and OBF to seek medical attention for my child, or myself, if in the judgment of the staff members it should be necessary. In the event my child should require medical attention and /or treatment during the course of any activities and after a reasonable attempt has been made to contact a parent or guardian, I hereby give permission to any hospital, physician, and/or other appropriate health care provider to undertake any form of medical treatment considered necessary or appropriate by such provider.
I agree to the use of my or my child's image or likeness in promotional materials for OBA or OBF including, but not limited to, newspaper ads or articles, brochures, social media, OBA website, and video for social media or television. If you do not agree to this portion of the Release of Liability please make sure to click on the photo opt out option under the waiver and release section of your parent portal. Electronic Signature Agreement. I am signing this agreement electronically. I agree this electronic signature is the legal equivalent of my manual signature on this agreement.
I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.
