You must be at least 18 years old to register for an account. By creating an account, you confirm that you are a parent or legal guardian and that you accept full responsibility for the use of this account by your child. You agree to supervise your child’s use of the service and to ensure that all activity complies with our Terms of Service and Privacy Policy.

By creating an account with the Universal Conservatory of Ballet (“UCB”), I acknowledge and agree to the following:

Assumption of Risk – I understand that participation in dance, fitness, and related activities involves physical activity and carries potential risks, including injury. I voluntarily assume all such risks for myself or my child.

Release of Liability – I release and hold harmless UCB, its owners, directors, instructors, employees, contractors, and volunteers from any liability for injury, loss, or damage arising from participation in UCB programs, whether caused by negligence or otherwise.

Medical Authorization – In the event of an emergency, I authorize UCB staff to obtain medical treatment for me or my child and accept full financial responsibility for any related costs.

Photo/Video Release – I grant permission for UCB to use photos or videos of me/my child for promotional purposes in print and online. (If you do not consent, please notify UCB in writing.)

Policies – I agree to follow all UCB rules, policies, and safety guidelines as published on the website or otherwise communicated.
By registering and creating an account, I certify that I have read this Agreement & Liability Waiver, understand its contents, and agree to be bound by its terms.
