Classes need to have a minimum of 5 registered dancers per class. If a class has 4 or less dancers it will be cancelled and you will be asked to find another class.

Waiver, Release, and Assumption of Risk Form

I hereby grant permission for My Child to attend the Wanna Dance Dance Camp and/or classes (together referred to herein as the “Camp”). My child and I are familiar with, and knowingly and voluntarily accept, any and all risks associated with attending the Camp. I acknowledge that My Child’s participation in the Camp is wholly voluntary, and I represent that I have the authority to sign this document on My Child’s behalf.

I specifically assume all risks and hazards associated with My Child’s participation in the Camp including, but not limited to, any and all risks associated with the COVID-19 virus and any variants of that virus (collectively referred to herein as “COVID-19”). I understand that My Child will be associating with staff and other children and may contract COVID-19, as well as other viruses and diseases, through My Child’s participation in the Camp. I understand and voluntarily assume the risk that My Child and/or anyone from my family may acquire COVID-19 from attending, and that COVID-19 may subsequently be transmitted among and to my family members and others.

While instruction and reasonable supervision will be provided, staff cannot ensure My Child’s safety. Accidents and injuries happen, and it is impossible to eliminate the risk that My Child will suffer an injury or illness.

I certify that My Child is in good health, has no fever, and has no current issues that make it unsafe for My Child to participate in the Camp, which does not have a medical professional on staff. I will notify Wanna Dance and not send My Child to the Camp if My Child develops a fever or illness or tests positive for COVID-19. I acknowledge that My Child and I are responsible for ensuring that he or she takes any necessary medication. In the event of a medical emergency, 911 will be called and I will be responsible for any and all medical treatment and related costs.

To the fullest extent permitted by law, I hereby agree on behalf of myself and My Child to waive, release, and discharge any and all claims, causes of action, damages, and rights of any kind against Wanna Dance, Samantha Goldenberg, and all of Wanna Dance's instructors, agents, representatives, and volunteers (the “Released Parties”) arising from or relating in any way to any damage, injury, trauma, illness, loss, unwanted contact, harassment, disability, dismemberment, or death that may occur to My Child, me, or my household members—whatever the cause—due to My Child’s participation in the Camp. This includes, without limitation, any claim arising from the negligence of the Released Parties.

I agree on behalf of myself and My Child to defend, hold harmless, and indemnify the Released Parties from any and all claims, demands, damages, liabilities, expenses (including, without limitation, any attorney's fees and costs), actions and/or judgments made or brought by any party that may arise out of or in connection with My Child’s participation in the Camp. This includes, without limitation, any and all claims for personal or bodily injuries, death or property damage, whether caused by the negligence or carelessness of the Released Parties or otherwise, except to the extent caused by the Released Parties’ gross negligence or intentional misconduct.

By signing up to attend the Camp, I agree on behalf of My Child to irrevocably grant to Wanna Dance — and to their employees, agents, and assigns — the absolute right and permission to use photographic portraits, pictures, digital images, right to take my child’s photograph, record my child, or use my child’s image, silhouette, and/or other reproductions of his/her physical likeness, as well as his/her voice and/or words, for all purposes related to marketing and promotion of Wanna Dance, across all media, without payment or any other consideration.

I hereby waive any right that I may have to inspect and/or approve the finished product or the copy that may be used in connection therewith, wherein My Child’s likeness appears, or the use to which it may be applied. These rights are granted in perpetuity. You hereby certify and represent that you are of at least 18 years of age and have read the foregoing and fully understand the meaning and effect thereof.

There is no food or drinks allowed inside the building. Please finish eating outside, before entering the building.

There is no food in class. There is no gum in class. There is no candy in class. Only water is permitted in classes.

Please respect our space and keep it clean!

Teachers may physically touch dancers to adjust body placement during instruction.

Tuition is collected on a semester basis. 1st Semester tuition is due in full upon registration. 2nd Semester tuition is due upon registration. Any camp or Summer Tuition is due in full upon registration. Team payments & private lesson payments are due on the 15th of the month. **If your bank account or credit card is invalid, or is declined, you will receive an email letting you know that you have 5 days to update your account. **A $50.00 late fee will be assessed to your account if you do not pay your tuition on time, or do not update your account with a valid form of payment within 5 days. After 10 days, another $50.00 late fee will be assessed to your account. After 15 days, another $50.00 late fee will be assessed to your account, and so on until payment is received.

Wanna Dance allows 2 make up classes per session. Dancers are allowed to take another class in his/her designated level, unless given written permission to attend another level. Classes need to be made up prior to the 10th week of instruction in a session, as classes will be focussing on Recital routines.

CLASS CHANGES AND DROPS
Wanna Dance strives to provide you with the highest quality of dance instruction and fun! If for some reason you are not satisfied, we will arrange for the following:
●Transfer to another class in the same session (given director approval)
●Refund: A full refund (plus a processing fee of $18) will be issued only if we are notified before the second class

Otherwise, we will only issue refunds due to medical reasons. A pro-rated refund plus a service charge of $18 will be assessed. A doctor’s note is required. Refund requests must be submitted in writing via email: WannaDanceLA@gmail.com.
Wanna Dance will ask you to enroll in a different class, should we feel your dancer
is better placed in another class.

If you need to cancel a class, you must do so before the class begins in order to receive a refund (plus a processing fee of $18). After the second week of classes, refunds will only be given if a child is physically unable to participate in a class, and a doctor's note is required.

We do not pro-rate or offer refunds for late enrollment, vacations, illness, missed classes, etc.

All deposits are non-refundable.

Dance Camp Policy:
Camps are first come, first serve. Waiting list participants must secure a spot with payment in full.

Cancellation Policy:
11 or more days prior to START OF CAMP WEEK: Refund, minus $75 fee
10 days prior to START OF CAMP WEEK or less: Credit only, minus $75 fee

There are no make-ups, refunds, or credits if the studio closes for an act of gd/nature.

All students are expecting to come into class independently, unless they are enrolled in out Tiny Tots classes.

Parents are to wait outside of the classroom. Tiny Tots and First Step parents are expected to wait close by, so they can be called to assist their dancer if he/she needs to use the restroom.

Classes are closed to parents unless Wanna Dance has specifically invited you to attend and observe a class.

Please wait outside and do not crowd the hallways.

There is no drop off or parking in the back alley!

Please find a legal parking space and walk your dancer to and from the front entrance.

All photographs and videos taken of Wanna Dance dancers can be used for the purposes of publicity. These photos may be used for program brochures, media productions, advertisements, posters, websites, social media, news articles, etc. We often take pictures and post on social media. You are agreeing that we are free to use your child's picture on our social media platforms.

There is a $35 Registration Fee due each year in the Fall or at the time of your first registration.
