I acknowledge that I have received, reviewed, or have access to The Isabelle School of Dance, Inc.’s policies, available at www.isabelledance.com/policies , and agree to abide by all studio rules and policies.

I understand that registration and participation constitute a financial commitment. I agree to pay all tuition, fees, and charges associated with my and/or my child(ren)’s enrollment.

I understand that tuition is not refundable except as expressly stated in studio policies, and missed classes due to illness, vacation, weather closures, or scheduling conflicts do not relieve financial responsibility.

I understand that withdrawal must follow the studio's official withdrawal policy and that tuition may remain due after withdrawal based on policy terms.

I understand that failure to pay tuition or fees may result in late fees, class suspension, or collection actions.

I authorize The Isabelle School of Dance, Inc. (“ISD”) staff to administer basic first aid to me and/or my child(ren) in the event of injury or medical emergency.

I authorize ISD to contact emergency medical services and consent to transportation by ambulance if deemed necessary.

I understand that ISD staff are not medical professionals and cannot diagnose or treat injuries.

I assume full financial responsibility for any medical care, treatment, or transportation provided to me or my child(ren) as a result of participation in ISD activities.

The Isabelle School of Dance, Inc. (“ISD”) is committed to providing a safe, respectful, and inclusive environment for all students and families.

I agree that I and/or my child(ren) will behave in a respectful and appropriate manner toward instructors, staff, and other students.

I understand that ISD reserves the right to suspend or dismiss any student or family whose behavior is disruptive, unsafe, or detrimental to the studio environment.

I understand that dismissal may occur without refund.

I understand that The Isabelle School of Dance, Inc. (“ISD”) may photograph or record students during classes, rehearsals, performances, competitions, camps, and events.

I grant ISD the irrevocable right and permission to use my and/or my child(ren)’s image, likeness, voice, and performance in photographs, videos, recordings, or other media, in any format now known or later developed, for lawful purposes including marketing, advertising, social media, website content, promotional materials, and publications, without compensation.

I understand that all such media shall remain the sole property of ISD.

I waive any right to inspect or approve the final use of such media.

I understand that I may submit a written request to opt out of media use, and ISD will make reasonable efforts to honor such requests, though ISD cannot guarantee exclusion from group photography or public event recordings.

I acknowledge that dance and physical activity involve inherent risks of injury, including but not limited to muscle strains, sprains, fractures, illness, and other physical harm. I voluntarily assume all risks associated with participation in classes, rehearsals, performances, competitions, camps, conventions, and related activities conducted by or associated with The Isabelle School of Dance, Inc. (“ISD”).

In consideration of participation, I, on behalf of myself and/or my child(ren), hereby release, waive, discharge, and hold harmless ISD and its owners, officers, instructors, employees, independent contractors, agents, volunteers, and guests from any and all claims, demands, liabilities, or causes of action arising out of or related to participation in ISD activities, except in cases of gross negligence or willful misconduct.

I understand that instructors may be employees or independent contractors and are covered under this agreement.

If signing on behalf of a minor, I certify that I am the parent or legal guardian of the participant and have full authority to execute this agreement on their behalf. I agree to indemnify and hold harmless ISD from any claims brought by or on behalf of the minor participant.

This agreement shall be governed by the laws of the State of New York. Any legal action shall be brought exclusively in the courts of Rensselaer County, New York.

If any portion of this agreement is held invalid, the remaining portions shall remain in full force and effect.
