By creating an account, you acknowledge that you are the parent or legal guardian of the student(s) you are registering. You must be at least 18 years of age to register. You also agree that all information provided is accurate and that you are responsible for all tuition, fees, and account activity.

By signing this document, I release Aria Performing Arts and their directors, owners, students, teachers, staff, employees, volunteers, associates from any liability or claim that I or my representatives may have against Aria Performing Arts with respect to any bodily injury, personal injury, illness, death, or property loss or damage that may result from my participation at Aria Performing Arts.

I voluntarily release and forever discharge and hold harmless Aria Performing Arts from any and all claims or demands for damages, loss of services, costs and expenses, injuries, attorney fees, and any other call for reparation from any and all injury to me or my property arising in any way from my participation in dance classes, camps, intensives, workshops, performances, troupes, the use of equipment or facilities, and any activities associated with Aria Performing Arts.

I fully understand that Aria Performing Arts instructors, agents, and employees are not physicians or medical practitioners of any kind. With the above in mind, I hereby release and grant permission to the Aria Performing Arts Staff to render temporary first aid to my child in the event of any injury or illness, and if deemed necessary by the Aria Performing Arts Staff to call emergency services to seek medical help.

I acknowledge and consent to allow Aria Performing Arts to use:
photos and/or videos of my child/myself in publications (such as flyers/brochures),
advertisements (such as newspapers or online ads), our studio’s website, or on social
media sites (such as the studio’s facebook or instagram account). I understand that in
some cases, my child and/or my name may be used. I further understand no financial
compensation will be for use of these photos/videos.

Students under the age of 10 must be walked up the stairs into the studio space if being dropped off for classes. The studio or its representatives do not take responsibility for students before or after their scheduled classes. It is the responsibility of the parent to ensure their child is picked up and dropped off in a timely manner. In the case of an emergency or unavoidable delay, please contact the studio immediately to inform us of the situation.
