Liability Release

RELEASE BY STUDENT, PARENT OR LEGAL GUARDIAN
The undersigned as parent or legal guardian of the students enrolled at Ozark Dance Academy and their heirs, legal representatives, and assigns, do hereby give my permission for said student to participate in any and all classes, performances and activities sponsored in conjunction with the Ozark Dance Academy.
Said Student and Guardian do further expressly release and discharge Ozark Dance Academy, Abigail Ellingsworth and Sierra Leeper both individually and doing business as Ozark Dance Academy, and all teachers, other students, and persons affiliated therewith from any and all claims of any nature for injury or loss which may result from said Student’s participation in such activities.
Said Students and Guardians do further stipulate and agree to indemnify and hold harmless Ozark Dance Academy, the heirs, successors and assigns against loss from any claims, demands or actions that may be brought by the Student or any other person or persons on account of any damages of any character resulting in any way from any accident or injuries which said Student may sustain as a result of participating in such
activities.

Medical Release

MEDICAL AUTHORIZATION
The undersigned, as parent or legal guardian of the student enrolled at Ozark Dance Academy whose do hereby give my express authorization to Ozark Dance Academy, Abigail Ellingsworth and Sierra Leeper, and their associates, to obtain medical treatment for said student in the event of an emergency while said student is under their supervision; this authorization does not limit the aforesaid in their selection of a medical provider nor does it place any legal burden upon the aforesaid to seek any medical attention for said student, but the aforesaid may select whatever medical provider they find suitable in their judgment given the student’s condition, location, and any other factor. This authorization shall expire August 31, 2026.
I do further release and discharge said Ozark Dance Academy, Abigail Ellingsworth and Sierra Leeper both individually and in doing business as Ozark Dance Academy and all directors, and members thereof, from all liability from any accident, injuries or damages which may result from such medical treatment.
