I hereby grant The Converge Dance Academy, its representatives, staff, and affiliated partners, the right to take photographs and/or video footage of my child or myself during classes, rehearsals, performances, events, and other studio-related activities.

I authorize The Converge Dance Academy to use such images and recordings for the following purposes:
-Social Media (Facebook, Instagram, etc.)
-Website and Online Promotions
-Print Materials (Flyers, Posters, Programs)
-Studio Displays and Marketing

I understand that:
-These materials may be used without further notice or compensation.
-The images and videos will never be sold or given to third parties.
-I may request in writing to withdraw consent at any time.

I give permission for my child/myself to be photographed or filmed for the purposes listed above.

I/we realize that participation in dance classes and activities could involve some possible personal injury (with bruises being the most likely injury and broken bones or other more serious physical injuries also being possible.) Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by The Converge Dance Academy LLC.
I/we agree to release and hold harmless The Converge Dance Academy LLC including its teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold The Converge Dance Academy LLC liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I/we
agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by The Converge Dance Academy LLC.
I understand that The Converge Dance Academy LLC is a licensed and insured business. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to the Owner, Director, Instructor or Staff member as soon as possible.
Further, I/we understand the physical demand of this activity and the practice required for its development and mastery. As a consideration for my own safety and enjoyment, as well as that of other students, I/we commit to dedicate necessary practice of the instructions and techniques given to me in class.
