RELEASE OF LIABILITY:
I hereby authorize Studio 7 to use photos/videos of my dancer related to our experiences with Studio 7. I understand this information may be used on our website and in publications, including electronic publications, audiovisual presentations, promotional literature, advertising, community presentations, letters to area legislators and media and/or other similar ways.

My consent is freely given as a public service to Studio 7, without expecting payment. I release Studio 7 and their respective employees, officers and agents from any and all liability which may arise from the use of such news media stories, promotional materials, written articles, videotape and/or photographs.

I understand that I can revoke this release any time in writing and that the use of any of my photos or other information authorized by this release will immediately cease.

MEDICAL EMERGENCIES:
In the event of an emergency related to injury/illness, the parent/guardian of the affected student(s) will be contacted immediately. In the event that the parent/guardian cannot be reached, the parent/guardian of the student authorizes the Studio 7 Staff, as well as the medical staff at a nearby medical center, to make decisions regarding the emergency care or treatment of their student(s), including seeking and approving medical treatment for non-emergencies.

ASSUMPTION OF RISK:
I understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child’s participation with Studio 7, and I voluntarily assume the risks associated with such participation. I understand that, as the parent(s) or guardian(s) of the participant, I am assuming full risk of injury arising from the use of Studio 7 facilities.

PAYMENT POLICY:
Upon accepting and submitting this policy, I have understood and agreed to the terms below.

I understand that payments can be made to the Studio 7 front desk by cash or check to Studio 7 after fees are posted on the first of the month and before the fourteenth of the month. Payments can also be made manually by credit card of ACH/EFT transaction through this Studio 7 Parent Portal between the first and the fourteenth of each month.

I understand that on the fifteenth of each month, the credit card saved on my Studio 7 Parent Portal will be charged for all outstanding balances plus a 3% credit card surcharge fee. If a bank account is saved on file, the bank account will be charged instead of the credit card for all outstanding balances.

All clients of Studio 7 are required to save credit card or bank account information to their Studio 7 Parent Portal. I understand that any transactions made via credit card will incur a 3% credit card fee.

I have received or will obtain a copy of Studio 7 policies and will take the responsibility to carefully read and follow the rules and policies therein. I understand that Studio 7 does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc.
