Acknowledgment of Risk
I understand that participation in online or in-person dance classes, workshops, rehearsals, and performances with the School of Dance Arts (“SODA”) involves inherent risks, including physical injury, illness, or other unforeseen incidents.
Release of Liability
I voluntarily assume all risks associated with participation and, on behalf of myself (or my child, if under 18), my heirs, and assigns, release and hold harmless SODA, its instructors, staff, and affiliates from any claims, demands, or damages arising from participation.
Medical and Safety
I certify that I (or my child) am physically able to participate in dance activities and will follow all safety instructions provided. I understand that SODA is not responsible for medical care or costs.
Media Release
I consent to the use of photos or videos of me (or my child) for promotional or educational purposes by SODA, without compensation.
Acknowledgment
By agreeing to this waiver, I acknowledge that I have read, understood, and agree to these terms. If the participant is under 18, I confirm that I am the parent or legal guardian and give permission for my child to participate.
