Trial classes are available for $15 per student and are limited to one trial per class style/program.

After the trial, you may continue attending at our regular drop-in rate of $20 per class, or choose to enroll for the full month/session at a discounted rate.

Trial class fees are due before or at the time of class (online or in person).

All trial and drop-in payments are non-refundable and do not guarantee a spot in the class unless you enroll.

To support safety, focus, and freedom of movement, all dancers are expected to follow the dress code for their class type:

Tots/Creative Movement:
Leotard (any color), tights, and ballet slippers, tap shoe if required
Hair pulled back neatly
Optional: tutu or dance skirt

Ballet:
Black Leotard, tights, and canvas ballet shoes
Hair in a bun and secured off the face
Boys:
Fitted shirt, men/boy ballet tights with a dance belt or fitted joggers

Jazz / Hip Hop / Contemporary:
Fitted top or leotard, leggings or dance pants/shorts
Jazz shoes or half soles
Hip hop: Clean indoor-only sneakers (not worn outside)
No jeans, skirts, or loose clothing

Tap:
Comfortable top, leggings or pants
Tap shoes
Hair off the face

Musical theater:
comfortable clothing
Clean indoor-only sneakers (not worn outside)

All Classes:
No jewelry (stud earrings are okay)
Bring a water bottle

We encourage self-expression within the guidelines, but all attire should allow instructors to see alignment and movement clearly. If you have questions about what’s appropriate, just ask — we’re happy to help!

I/we realize that participation in dance classes and activities could involve some
possible personal injury. Despite precautions, accidents and injuries may occur. By
signing this release form, I/we (the dancer and parent/guardian) assume all risks
related to the use of any and all spaces used by Bloom Dance Movement.
I/we agree to release and hold harmless Bloom Dance Movement including its
teachers, dancers, staff members, and facilities used by both entities from any cause
of action, claims, or demands now and in the future. I/we will not hold Bloom Dance Movement
liable for any personal injury or any personal property damage,
which may occur on the premises before, during or after classes. Furthermore, I/we
agree to obey the class and facility rules and take full responsibility for my/our
behavior in addition to any damage I/we may cause to the facilities utilized by
Bloom Dance Movement.
I understand that Bloom Dance Movement is a licensed and insured business. In the
event that I/we should observe any unsafe conduct or conditions before, during or
after my/our classes, I/we agree to report the unsafe conduct or conditions to the
Owner, Director, Instructor or Staff member as soon as possible

YES- I consent. I grant permission for my child to participate and appear in video or audio recording, films, photographs, written articles, or on websites and social media sites.

Acknowledgment of Tuition and Late Fee Policy

Tuition must be paid in full by the due date specified by the studio.
Tuition is non-refundable and non-transferable unless otherwise stated.

A late fee of $25 will be charged if payment is not received within 5 days of the due date.
Continued non-payment may result in suspension from class participation until the balance is paid.

Tuition is based on enrollment, not attendance. Missed classes do not qualify for tuition adjustments or refunds.
Make-up classes are available, must be used within 30 days of missed class, and a student must be currently enrolled in order to use make-up class.

I acknowledge that I have read, understood, and agree to comply with the tuition and late fee policies set forth by Bloom Dance Studio.I understand that it is my responsibility to stay informed about all payment deadlines and studio policies. By enrolling in classes, I agree to these terms and waive any claims against Bloom Dance Movement related to tuition or late fee disputes.
