-I/We acknowledge having received the studio policies, have reviewed them in their entirety and acknowledge that we understand them and promise to abide by them.
-I/We understand the risks related to dance.
-I/We understand our payment obligations and that payments received after its due date will be subject to a late charge of $15 per month.
-I/We understand that all payments and fees are non-refundable.
-I/We understand any costumes ordered on my child's behlaf must be paid for once they have been ordered.
-I/We authorize LEGACY DANCE CO to use images on the studio website, social media, brochures and other promotional materials.
-I/We understand I am/We are responsible for our own property.
