Parents/guardians must be 18 years or older to create an account and register a dancer with Dance Nation. By creating an account, I agree to provide accurate family, student, medical, emergency contact, and payment information.

TUITION PAYMENT, ENROLLMENT, BILLING, AND ACCOUNT STANDING
I understand that if my child is enrolled in a program with recurring monthly tuition, I am required to have a valid, working credit card on file. I understand that my child is continuously enrolled in the program and will incur recurring monthly tuition charges until I submit an official Dance Nation class stop request.
If I am stopping a class with recurring monthly tuition, it must be submitted in writing through Dance Nation’s official class stop request process on or before the 20th day of the current month. If I stop a class after the 20th of the current month, I will not receive credits and/or refunds for the remaining classes in the current month.
I understand that Dance Nation does not issue refunds. All sales are final for any product and/or service purchased and/or provided by Dance Nation.
I understand that Dance Nation does not give make-up classes, credits, and/or refunds for missed classes, holidays, vacations, illness, weather closures, family scheduling conflicts, voluntary absences, or any other reason, unless otherwise stated by Dance Nation in writing.
Tuition price has been calculated for the full dance season and divided into monthly payment installments. Monthly tuition will remain the same whether there are 3, 4, or 5 weeks in the month. Tuition is not based on the number of classes in each individual month.
If Dance Nation must cancel a class for any reason, and the class is not offered a minimum of 32 lessons for the year, a make-up opportunity will be provided. Make-up classes may be provided virtually in the event the studio is closed due to unforeseen circumstances or inclement weather.
I am responsible for making timely payments of all balances due on my Dance Nation account. From the date of registration forward, my entire account balance shall be due on the assigned due date. For recurring monthly tuition, tuition will be due on the 1st of each month.
I understand that monthly payment amounts may vary as classes are added or dropped and as other charges, fees, or payments are applied to my account.
Registration fees, tuition payments, costume fees, recital media/package fees, choreography deposits, choreography balances, admin fees, competition fees, private lessons, camps, events, and merchandise are non-refundable unless Dance Nation states otherwise in writing.
Fees for other products and/or services shall be paid at the time of purchase and/or registration.
I understand that Dance Nation may attempt to process outstanding balances using the payment method on file. Payments will be processed with the payment method/information I have provided through registration and/or my Dance Nation account.
I acknowledge that this authorization will remain in effect until I notify the Dance Nation business office in writing that the authorization should be terminated.
OVERDUE PAYMENTS, FAILED PAYMENTS, AND ACCOUNT STANDING
I understand that all Dance Nation balances are due by the assigned due date. Families are responsible for keeping a valid, working payment method on file and updating billing information before any scheduled payment date.
If a payment fails or an account balance becomes overdue, the balance must be corrected within 7 days.
If the balance is not corrected within 7 days, a $10 late fee may be added to the account.
If the balance is not corrected within 14 days, an additional $15 administrative fee may be added to the account to help cover the staff time required to follow up, track, and manage the overdue balance.
If the balance is not corrected within 21 days, the student’s participation may be paused until the account is paid in full.
If Dance Nation staff must repeatedly follow up regarding failed payments, overdue balances, declined cards, expired cards, missing payment methods, or unresolved balances, additional administrative late fees may be added to help cover the staff time required to manage the account.
Dance Nation reserves the right to pause a student’s participation in classes, rehearsals, private lessons, choreography, camps, competitions, performances, recital, dress rehearsal, and/or special events until the account is current.
Accounts with repeated failed payments, repeated overdue balances, or repeated late payments may be required to pay future charges in advance before participation is allowed.
Dance Nation reserves the right to remove a student from enrollment, refuse future registration, or discontinue services for families with repeated failed payments, repeated overdue balances, unresolved balances, or a history of non-payment.
All overdue balances must be paid in full before a dancer may participate in recital, dress rehearsal, competitions, choreography, private lessons, camps, intensives, special events, or any additional Dance Nation programming.
I will be responsible for all costs incurred for collection of any delinquent payments, including but not limited to collection fees, attorney fees, and court costs.
RECITAL MEDIA PACKAGE FEES AND COSTUME EXPENSES
Dance Nation may hold two recital/performance opportunities per Fall-Spring Season, including Merry Movement and Spring Recital.
I understand that Merry Movement costume fees will be billed with October tuition and will be due October 1st.
I understand that the Merry Movement Recital Media Package will be billed with November tuition and will be due November 1st.
I understand that Spring Recital costume fees will be billed with February tuition and will be due February 1st.
I understand that the Spring Recital Media Package will be billed with March tuition and will be due March 1st.
I understand that Dance Nation does not give credits or refunds for recital costumes, recital media package fees, and/or any other recital or costume fees or expenses.
If my account remains overdue at the time of recital, my dancer will not be eligible to participate in dress rehearsal or recital.
ELECTRONIC SIGNATURE AGREEMENT
By selecting the "I Accept" checkbox, I am signing this agreement electronically. I agree this electronic signature is the legal equivalent of my manual signature on this agreement.
I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms. Dance Nation reserves the right to modify the terms of this agreement with written notice.

ASSUMPTION OF RISK. WAIVER OF LIABILITY
As legal guardian of the minor named persons, I recognize that potentially severe injuries, including permanent paralysis or death can occur in sports or activities involving height or motion, including but not limited to training equipment, dance class, inflatables, gymnastics, tumbling, clinics, camps, private lessons, birthday parties, birthday party guests, open gym, camps, play time, and field trips. Being fully aware of these dangers, I voluntarily consent to the aforementioned persons participating in any and all programs at Dance Nation and I ACCEPT ALL RISKS associated with that participation.
In consideration for allowing my child to use this facility, I, on my own behalf and the behalf of my child and our respective heirs, administrators, executors, and successors, hereby COVENANT NOT TO SUE and FOREVER RELEASE Dance Nation, its officers, directors, shareholders, employees, or other representatives, whether paid or volunteer, from all liability for any and all damages or injuries suffered by my child while under the instruction, supervision, or control of Dance Nation.
I also understand that it is the parent's responsibility to warn the child about the dangers of dance, exercise, tumbling, gymnastics, and injury. Dance Nation will only warn the child through safety messages, teaching style, and progressions.
I further acknowledge, understand, and appreciate that participation may result in possible exposure to and illness from infectious diseases, including but not limited to MRSA, Influenza, and COVID-19. I knowingly and freely assume all risks, both known and unknown, associated with infectious diseases, even if arising from the negligence of the release of others, and assume full responsibility for participation and exposure.
PERMISSION FOR EMERGENCY MEDICAL TREATMENT / MEDICAL INSURANCE
I confirm that my child is in good health, and I have medical insurance coverage for my child and will continue to provide coverage while he/she is enrolled.
I fully understand that Dance Nation staff members are not physicians or medical practitioners of any kind. With the above in mind, I hereby release Dance Nation staff members to render temporary first aid to my child in the event of any injury or illness, and if deemed necessary by Dance Nation staff, to seek medical help, including calling an ambulance for said child.
Additionally, I hereby agree to individually provide for all medical expenses that may be incurred by my child as a result of any injury sustained while participating at Dance Nation.
USE OF IMAGE
I understand and give permission for photo or video images of my child and/or myself to be used without names as deemed appropriate for the promotion of Dance Nation.
COMMUNICATION
I understand and give permission for Dance Nation to contact me through texts, phone calls, emails, etc. I am required to have a current and up-to-date phone number, email address, physical address, and emergency contact on file while my child is enrolled in any Dance Nation program.
