I agree that I am authorized to enroll the student I am creating an account for.

I Hereby enroll my child in class(es) at Wanda Kay’s School of Dance, I understand that my child will be involved in physical activities that entail a risk of injury. While exercising reasonable caution, I release Wanda Kay’s School of Dance and its employees from any liabilities which may arise while at the dance studio.

I hereby give permission for my child's picture while at Wanda Kay’s School of Dance to be used on Social Media, Online, or in the local newspaper as an advertisement for Wanda Kay’s School of Dance ONLY.

I hereby give permission to Wanda Kay’s School of Dance staff to seek medical attention in case of a medical emergency when a parent or guardian is unable to be reached.
