I agree that I am authorized to enroll the student I am creating an account for.

I Hereby enroll my child in class(es) at Limelight Dance, I understand that my child will be involved in physical activities that entail a risk of injury. While exercising reasonable caution, I release Limelight Dance and its employees from any liabilities which may arise while at the dance studio.

I hereby give permission for my child's picture while at Limelight Dance to be used on Social Media, Online, or in the local newspaper as an advertisement for Limelight Dance ONLY.

I hereby give permission to Limelight Dance staff to seek medical attention in case of a medical emergency when a parent or guardian is unable to be reached.
