I acknowledge the Shoreline handbook as posted online at www.shorelinedance.net
I grant Shoreline Dance Studio the right to take photos/videos of my child in connection with the dance studio.
I authorize Shoreline to use and publish photos in print and or electronically.
I on behalf of myself and/or my minor child hereby release Shoreline Dance, its officers, agents, and employees from all claims of injury or illness which may be sustained by my child or myself while participating in dance classes, rehearsals, performances or other dance related activities. I also release the Sheldon Theatre from all claims of injury or illness which may be sustained by my child while participating in dress rehearsals and recital performances.
