Legal Release
I, the adult applicant, or I, the parent or legal guardian of the applicant(s) listed above, hereby have approval of the applicant's participation in all Agape Dance programs and activities. Any child dropped off at the studio is to always remain in the studio. Any property brought into Agape Dance/M & W Construction/Wimberly Warehouse is the responsibility of the parent/legal guardian or student. Any items left during performances or classes will attempt to be placed in a centralized lost and found box but is not the responsibility of Agape Dance or M & W Construction/Wimberly Warehouse. I do waive, release, absolve, indemnify, and agree to hold harmless the organizers, sponsors, supervisors, participants, property owners and persons involved in the operation of Agape Dance programs and M & W Construction/Wimberly Warehouse for any claims arising out of injury, or other loss to named applicant(s) or any member of his/her family whether as a participant in the activities or as a spectator. I understand I am responsible for all medical bills that may occur because of an injury while on the property, at a performance or in dance class. Injury or illness to anyone on the property, regardless of whether they are in dance class or not, is the responsibility of the parent or guardian.
I also give permission for Agape Dance to take photos of me or my child/children to use for purposes of promoting the program. If any child exhibits behavior that is dangerous to himself/herself or to other students Agape Dance reserves the right to remove the child from the studio.
I understand every effort will be made to contact me, the contact person in case of medical emergency. If we cannot be reached, I give my consent for the emergency room physician to treat myself, my child, or my family.
