1. Assumption of Risk
I acknowledge that participation in dance and/or yoga classes and related activities (the “Activities”) at Studio Ifé involves physical movement and the risk of injury, including but not limited to strains, sprains, falls, or other physical injuries. I understand the nature of these Activities and confirm that I am (or my child is) in good health and proper physical condition to participate safely.
I voluntarily choose to participate (or allow my child to participate) in these Activities and fully accept and assume all risks, known and unknown, even those arising from the negligence of Studio Ifé or its instructors, staff, or representatives.
2. Medical Release
In the event of an emergency, I authorize Studio Ifé to obtain medical treatment for me or my child. I understand that I am responsible for all medical expenses incurred.
3. Waiver and Release of Liability
I hereby release, waive, discharge, and hold harmless Studio Ifé, its owners, instructors, employees, volunteers, and agents from any and all claims, demands, liabilities, costs, and causes of action that may arise from personal injury, property damage, or any other loss resulting from participation in the Activities, whether caused by negligence or otherwise.
4. Photography & Media Release
I grant permission to Studio Ifé Dance and Yoga to take and use photographs and/or video recordings of me and/or my child during classes, performances, or events for marketing and promotional purposes, including but not limited to social media, the website, flyers, and other promotional materials. I understand that no personal identifying information will be shared without additional consent.
5. Acknowledgment of Understanding
By signing below, I affirm that I have read this Assumption of Risk and Release of Liability form in its entirety, understand its contents, and voluntarily agree to its terms.
