In the event of a medical emergency, I authorize Mo' Motion Dance Academy to seek emergency medical treatment for my child. I understand that the studio will make every effort to contact me before taking this action.

I understand that tuition for classes at Mo' Motion is due monthly on the 1st of each month. A late fee of $25 will be applied for payments not received by the 5th. I acknowledge that I am responsible for payment regardless of student attendance unless I provide a 30-day written withdrawal notice.

Release of Liability
I, the undersigned, understand that participation in dance and movement activities involves physical exertion that carries the risk of injury. I agree to release and hold harmless Mo' Motion Dance Academy, its owners, instructors, staff, volunteers, and affiliates from any and all claims, liability, or causes of action arising out of or connected to participation in any class, event, rehearsal, or performance hosted by Mo' Motion Dance Academy.

I understand that Mo' Motion Dance Academy is not responsible for any lost or stolen items. I certify that the participant is in good health and physically able to participate in dance activities.

I hereby grant Mo' Motion Dance Academy permission to take and use photographs, video recordings, or digital images of the participant for promotional and educational materials including, but not limited to, social media, brochures, newsletters, press releases, websites, and internal documentation.
