LIABILITY WAIVER FORM



I acknowledge that I have voluntarily enrolled in dance classes at Raakhe Virse De Academy. I understand that dance classes involve physical activity and carry with them a risk of injury. I agree to assume all risks associated with participation in dance classes and any related activities.

I hereby release Raakhe Virse De Academy, its owners, directors, instructors, employees, and agents from any and all liability, claims, demands, and causes of action arising out of or related to my participation in dance classes and related activities, including but not limited to, personal injury, property damage, or wrongful death.

I agree to comply with all rules and regulations of Raakhe Virse De Academy and to follow the instructions of its instructors.

I understand that this waiver and release is binding on me, my heirs, executors, administrators, and assigns.

I understand that Raakhe Virse De Academy may take photographs and/or videos of me during dance classes or related activities. I hereby consent to the use and reproduction of any and all such photographs and videos by Raakhe Virse De Academy for promotional, advertising, or other purposes.

I further understand that Raakhe Virse De Academy may share such photographs and videos on its social media accounts, website, or other marketing materials. I hereby consent to such sharing and acknowledge that I will not receive any compensation or remuneration for such use.

I understand that Raakhe Virse De Academy does not carry health or accident insurance for its students. As such, I acknowledge that I am responsible for my own medical and/or dental expenses incurred as a result of any injury or illness suffered while participating in dance classes or related activities.

I certify that I am in good physical condition and have no medical or physical conditions that would prevent me from participating in dance classes or related activities.

Payment Schedule:

a. Tuition payments for dance classes are due after every four (4) classes.

b. For students who join within the first four classes of a session, payment is due for the remaining number of classes left in that session.

b. Payments are due on the first class following the completion of the fourth class.

c. Payments can be made in card (3% processing fee applies), or electronically via Zelle (rvdacademy@gmail.com) as agreed upon by the dance studio and the student or parent/guardian.

Late Payment Fee:

a. A late fee of $10 will be charged if the tuition payment is not made by the end of the second (2nd) week of classes.

b. The second week ends on the last day of classes of the second week. c. The late fee will be added to the outstanding balance and must be paid along with the late payment.

Accepted Payment Methods:

a. Check: Payments made by check should be made payable to RVD Academy and can be submitted in person.

b. Electronic Payment: Electronic payment options, such as online banking or payment processors, may be available. The specific details and instructions will be provided by the dance studio.

c. Illinois locations will only accept electronic payment.

Refunds and Withdrawals:

a. No refunds will be provided for missed classes or withdrawals after the payment has been made.

b. In case of withdrawal from the dance program, the dance studio should be notified in writing.

c. If a withdrawal notification is received after the tuition payment due date but before the end of the second week, the late fee will still apply.

Missed Classes:

a. Students can notify the dance studio before their first class of the month if they intend to miss more than 2 classes for the month to be eligible for a reduced rate.

b. $10 will be reduced for each missed class for individual members, and $7.50 will be reduced for each missed class for additional members within the same family who are also missing classes.

c. Students missing one class in a month are not eligible for a reduced rate.

Non-Payment Consequences:

a. Students who fail to make payment within a reasonable time frame may be denied further participation in dance classes until the outstanding balance is settled.

b. The dance studio reserves the right to take appropriate action, which may include the involvement of a collection agency, for non-payment of tuition fees.

Communication:

a. The dance studio will provide timely reminders and notifications about payment due dates and late fees through email, text messages, or any other preferred mode of communication specified by the student or parent/guardian.

b. It is the responsibility of the student or parent/guardian to ensure that the dance studio has accurate and up-to-date contact information.
