Waiver & Release of Liability, Assumption of Risk, and Photo Release Agreement

1. Waiver & Release of Liability
I, the undersigned, understand and acknowledge that participation in dance classes, rehearsals, performances, and related activities at Camarillo Dance Collective, involves inherent risks, including but not limited to all physical injury, illness, falls, strains, and or sprains.

I voluntarily assume full responsibility for any risks, injuries, or damages that I or my child may incur in connection with participation in any and all Studio activities. I hereby release, waive, discharge, and hold harmless Camarillo Dance Collective, its owners, employees, instructors, volunteers, agents, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, injury, damage, or death that may be sustained while participating in Studio activities, whether caused by negligence or otherwise.

2. Assumption of Risk
I understand the nature of the activities offered and confirm that I or my child am/is physically and mentally able to participate. I accept full responsibility for determining my (or my child’s) fitness to participate. I acknowledge that participation is voluntary and that I assume all associated risks.

3. Medical Authorization
In the event of a medical emergency, I authorize Camarillo Dance Collective to seek medical treatment for me or my child. I understand that I am responsible for any medical expenses incurred.

4. Photo & Video Release
I grant permission to Camarillo Dance Collective to take photographs and/or video recordings of me or my child during studio classes, events, or performances. I understand these images may be used in promotional materials, social media, websites, publications, or other media. I waive any right to compensation or ownership of the photos or recordings.

5. Acknowledgment
I have read and understood this Waiver & Release in its entirety. I understand that by signing this document, I am waiving certain legal rights, including the right to sue. This agreement shall be binding on me, my heirs, assigns, legal representatives, and successors.
