Waiver of Liability
By signing this waiver, I, the undersigned, acknowledge and agree to the following:
Risk of Injury
I understand that participation in dance classes and activities involves physical exertion, including but not limited to jumping, stretching, turning, and other movements that may pose a risk of injury. I acknowledge that injuries, including but not limited to sprains, strains, fractures, and muscle injuries, can occur during these activities.
Release of Liability
I voluntarily assume all risks associated with participating in dance classes and activities. I release and hold harmless the dance studio, its instructors, employees, contractors, and affiliates from any and all liability for injury, loss, or damage to personal property, whether caused by negligence or otherwise, resulting from my (or my child’s) participation in dance classes.
Medical Clearance
I confirm that the student is in good health and able to participate in dance activities. I understand that it is my responsibility to consult with a healthcare provider before starting any physical activity if there are any concerns about health or fitness.
Permission for Emergency Medical Treatment
In the event of an emergency, I give permission for the dance studio staff to seek medical attention for the student if I cannot be reached. I agree to assume responsibility for all costs associated with any medical treatment or transportation.
Photographs & Videos
I consent to the use of photographs, videos, or other media in which the student may appear being used for promotional purposes, including but not limited to the studio’s website, social media, and printed materials. I understand that the student’s name will not be used unless permission is granted.
Code of Conduct & Behavior
I agree that the student will adhere to the studio’s code of conduct, including respectful behavior towards instructors, staff, and fellow students. The studio reserves the right to dismiss any student from the class due to disruptive behavior, without refund, at the discretion of the instructor or management.
Payment & Refunds
I agree to pay the class fees in full according to the studio's payment policies. I understand that all fees are non-refundable after the first class, except in cases where the class is canceled by the studio. Makeup classes may be available based on studio policies.
