PARENT/GUARDIAN AGREEMENT & WAIVER: I acknowledge and agree to the following policies and requirements: 1.Tuition Policy Tuition is posted on the 1st of each month on the Dance Studio Pro Portal and is due by the 10th. Payment must be received by the 10th to avoid late fees. Payments are considered received only when funds are in hand; backdated checks will not be accepted as on-time payment. (But we will hold if requested till the 10th if turned in earlier). 2.Costume Policy All costume balances must be paid in full before any costumes are released to students. Costumes will not be sent home until full payment has been received. 3.Recital Eligibility Agreement I understand that all outstanding balances must be paid in full in order for my child to participate in the recital and purchase recital tickets. Students with unpaid balances will not be eligible to perform. By checking below, I confirm that I have read, understand, and agree to abide by all policies listed above.
