All statements below refer to any and all of the above referenced entities and apply to the legally registered company, Abundant Hope Christian Performing Arts. I recognize that there are certain inherent risks associated with participating in any physical activity such as ballet and dance classes. I give permission for my child to participate in all activities.

Parents will be contacted for all medical emergencies.

Regular Tuition Fees are due on the 1st day of each month and will be processed automatically through the bank account or credit card on file. A $15 Late Fee will be applied to any overdue balance as of the 7th of every month. Every additional week incurs another $15 fee. If you are more than one month behind on tuition, students will not be able to attend classes until balance is paid in full. If you prefer to pay with a check please give to Miss Berkleigh before the 1st of each of month.

For months with 5 class days rather than 4, an extra week of tuition (25% of the monthly fee) will be charged to cover the cost of the teacher salary for that extra week. For months when the studio is closed for a holiday, tuition will be prorated and you’ll only pay for the weeks we have classes. If you pay with a credit card, there will be an added convenience charge of 3.05% plus .35 for each transaction and for ACH payments there will be an added charge of 1.00% plus .20 for each transaction.

I give permission for the student's photo/image to be posted on Abundant Hope’s social media sites and other marketing items.

I release any person or agent working on behalf of Abundant Hope Christian Performing Arts who executes instruction in physical activities. I fully release Abundant Hope Christian Performing Arts for any injury, loss or damage arising out of my child's use of or presence upon the facilities of Abundant Hope Christian Performing Arts, whether caused by the fault of myself, my child, Abundant Hope Christian Performing Arts, instructors, or other third parties. I agree to indemnify and defend Abundant Hope Christian Performing Arts against all claims, causes of action, damages, judgments, costs or expenses, including attorney fees and other litigation costs, which may in any way arise from my or my child's use of or presence upon the facilities of Abundant Hope Christian Performing Arts. Any legal or equitable claim that may arise from participation in the above shall be resolved under Florida law. As the parent/legal guardian of this student, I have read and understand the above questions and acknowledge that by choosing "YES" and submitting this registration form, I am voluntarily agreeing to hold Abundant Hope Christian Performing Arts harmless and that I am surrendering certain legal rights.
