In consideration of being allowed to participate in any way at Premier Sports Lab, including but not limited to training sessions, practices, camps, and events (collectively, the “Activities”), I, the undersigned, acknowledge and agree to the following:
1. Assumption of Risk
I understand that physical activity, including sports performance training, involves inherent risks of injury, including but not limited to strains, sprains, broken bones, concussions, and in rare cases, paralysis or death. I voluntarily assume full responsibility for any risk of loss, damage, or personal injury that may be sustained as a result of my participation in the Activities.
2. Release and Waiver of Liability
I hereby release, waive, discharge, and covenant not to sue Premier Sports Lab, its directors, officers, employees, volunteers, agents, sponsors, and affiliates from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my child(ren) while participating in the Activities or while on the premises.
3. Medical Authorization
I authorize Premier Sports Lab to obtain emergency medical treatment as deemed necessary. I assume responsibility for all medical expenses incurred as a result of participation.
4. Indemnification
I agree to indemnify and hold harmless Premier Sports Lab and its staff from any loss, liability, damage, or costs incurred due to my or my child’s participation.
5. Acknowledgment of Rules and Conduct
I agree to follow all rules and instructions provided by Premier Sports Lab staff. I understand that failure to comply may result in termination of participation without refund.

I hereby grant Premier Sports Lab the right and permission to use photographs, video, or other media recordings of myself or my child taken during participation in activities at the facility or offsite events, for any lawful purpose, including but not limited to:
• Marketing and advertising
• Social media content
• Print materials
• Website content
• Promotional campaigns

I waive any right to inspect or approve the finished product wherein my likeness appears. I understand that I will receive no compensation for the use of such media.

I also understand that all images and recordings become the property of Premier Sports Lab and may be used indefinitely unless I revoke this permission in writing.
