I understand that this waiver will remain in effect for the duration of my (or my child’s) participation in activities at The Phoenix Ballroom Academy. I certify that I am over the age of 18 or am the legal guardian of the minor participant named above.

I understand and acknowledge that participation in dance activities involves physical movement and carries with it the risk of personal injury. I understand that such risks may include, but are not limited to, falls, sprains, muscle strains, broken bones, and other potential injuries.

I voluntarily assume full responsibility for any risk of injury, loss, or damage to person or property that may occur as a result of participation in classes, rehearsals, performances, or any other activity at or sponsored by The Phoenix Ballroom Academy.

In consideration of being allowed to participate in any way in programs or events at The Phoenix Ballroom Academy, I hereby release, discharge, and hold harmless the studio, its owners, employees, instructors, agents, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained by me (or my child), whether caused by negligence or otherwise.

In the event of an emergency, I authorize The Phoenix Ballroom Academy staff to secure from any licensed hospital, physician, and/or medical personnel any treatment deemed necessary for my (or my child’s) immediate care. I agree to be responsible for payment of any and all medical services rendered.

A 30-day notice is required for students not wishing to return to classes otherwise you are liable for payment for the following month. Tuition is due by the 1st of every month following registration. Payments made by cash or check may be hand delivered to the instructor or left in the studio office. Payments made after the 5th of the month will incur a $15 late fee. A $25 returned check fee will be applied to all NSF checks and is due before the next month's tuition. All fees will be charged to the credit card on file on the 1st of the month unless you have opted out of auto pay via email communication with the studio info@quickstepstudio.com There is no refund for missed classes, where applicable makeup classes will be offered. If you wish to drop your child from a class you must submit 30 day written notice of a withdrawal from class in order to avoid being charged for the next month's tuition.

I hereby grant permission to The Phoenix Ballroom Academy, its representatives, and authorized media partners to photograph, video record, and/or otherwise capture the image, likeness, or voice of me (or my child) during classes, performances, events, or other studio-related activities.

I understand that these photographs and recordings may be used for the following purposes, including but not limited to:

Promotional materials (flyers, posters, brochures)
Social media content (Facebook, Instagram, website, etc.)
Online advertisements or digital campaigns
Press releases and media coverage
Archival or educational purposes

I waive any right to inspect or approve the finished product or the use to which it may be applied. I understand that no royalty, fee, or other compensation shall be payable to me (or my child) for such use.
