I understand that dance training involves physical activity and carries the risk of injury. I release Maumelle Dance Academy, the staff, and it's affiliates from liability for injuries sustained while participating in classes, performances, or events. I authorize staff to obtain emergency medical treatment if necessary. I will inform my (or my child's) instructors of any medical conditions that could affect my (or their) dance instruction.
By checking this box, I acknowledge that I have read and understand the Maumelle Dance Academy 2026-2027 Studio Guidelines & Student/Parent Handbook. I agree to abide by all studio policies, including those related to tuition and automatic payments, attendance, dress code, costume and recital fees, recital participation, studio etiquette, holidays and studio closures, refunds, and all other policies outlined in the handbook. I understand that these policies are intended to provide a positive, safe, and organized experience for all students and families. I understand that it is my responsibility to review the handbook and stay informed of any future updates communicated by the studio. I grant Maumelle Dance Academy permission to use photos and videos of myself (or my children) taken during classes, rehearsals, and performances for educational and promotional purposes (including social media, print materials, and the studio website). I understand that no personal information will be shared. If I wish to revoke this permission, I will submit a written request to the studio.
