I understand that dance training involves physical activity and carries the risk of injury. I release Maumelle Dance Academy, the staff, and it's affiliates from liability for injuries sustained while participating in classes, performances, or events. I authorize staff to obtain emergency medical treatment if necessary. I will inform my (or my child's) instructors of any medical conditions that could affect my (or their) dance instruction.

I grant Maumelle Dance Academy permission to use photos and videos of myself (or my children) taken during classes, rehearsals, and performances for educational and promotional purposes (including social media, print materials, and the studio website). I understand that no personal information will be shared. If I wish to revoke this permission, I will submit a written request to the studio.
