BURLINGTON ACADEMY OF DANCE & ARTS
ENROLLMENT, PAYMENT AUTHORIZATION, WAIVER & RELEASE AGREEMENT
By enrolling at Burlington Academy of Dance & Arts ("BADA"), I acknowledge that I have read, understand, and agree to the following terms and conditions.
REGISTRATION & PAYMENT POLICIES
• Registration fees are non-refundable.
• Tuition is due on the 1st of each month. A grace period is provided through the 10th of the month.
• An active ACH/EFT bank account and/or credit card must be maintained on file at all times.
• BADA does not accept debit cards. While the registration system may allow debit cards to be entered, debit cards are not an approved payment method for BADA accounts. Families must utilize cash, check, ACH/EFT bank draft, or a credit card. ACH/EFT bank draft provides the same convenience as automatic payments while avoiding significant processing fees charged to BADA.
• If a debit card is entered into the system, BADA reserves the right to require replacement with an ACH/EFT bank account or approved credit card before registration is considered complete.
• If any balance remains unpaid after the 10th of the month, BADA is authorized to process payment using the payment method on file.
• A $25 late fee will be assessed on any unpaid balance after the 10th of the month.
• A $30 NSF/returned payment fee will be assessed for any returned check, declined ACH transaction, rejected payment, or chargeback.
• BADA reserves the right to suspend participation, withhold costumes, restrict performance participation, and/or cancel enrollment for accounts that remain delinquent.
DANCE YEAR ENROLLMENT POLICIES
(Applicable Only to 2026-2027 Dance Year Enrollments)
• Registration is not complete until all required registration payments have been received, including:
Registration Fee
August 2026 Tuition
May 2027 Tuition
• August 2026 tuition and May 2027 tuition are required at registration for all 2026-2027 Dance Year enrollments.
• August 2026 tuition will be billed and automatically processed using the payment method on file on the next business day following registration and is due upon receipt.
• May 2027 tuition will be billed and automatically processed using the payment method on file at the time of registration.
• May 2027 tuition is refundable only if written cancellation is received by August 15, 2026. After August 15, 2026, May 2027 tuition becomes non-refundable.
• Written notice is required to withdraw from classes. Withdrawal requests must be submitted in writing prior to the 1st day of the month. Families remain financially responsible for any month in which written notice was not received prior to the 1st.
• No refunds are provided for missed classes. Students may request a make-up class in a lower-level class of the same discipline with office and faculty approval. Requests must be made through the office during office hours at least 24 hours in advance.
COSTUME & PERFORMANCE FEES
(Applicable Only to Dance Year Performance Classes)
• Costume Deposit: $65 per costume due November 1, 2026.
• Costume Balance: Due February 1, 2027.
• Costume fees remain due if a class is dropped after November 15, 2026.
• Performance Fees Due April 1, 2027:
$65 per dancer cast in one evening of Spring Performance
$95 per dancer performing in both evenings of Spring Performance OR the Spring Ballet only
$125 per dancer performing in one evening of Spring Performance AND the Spring Ballet
$155 per dancer participating in both evenings of Spring Performance AND the Spring Ballet
SUMMER PROGRAM POLICIES
(Applicable Only to Summer Classes, Camps, Intensives, Workshops, and Special Events)
• Summer registration fees are non-refundable.
• Summer tuition must be paid prior to the first class, camp, workshop, intensive, or event.
• No withdrawals, refunds, prorating, drop-in rates, or make-up classes are available for summer camps, workshops, intensives, or special events.
• No refunds are provided for missed summer classes.
REFUND POLICY
• All tuition, registration fees, costume fees, performance fees, deposits, and other charges are non-refundable unless specifically stated otherwise herein.
• Account credit may be issued at the sole discretion of BADA.
PAYMENT AUTHORIZATION
Automatic / Recurring Payments
I authorize BADA to automatically charge the payment method on file for balances due on my account. Payment amounts may vary as classes are added or dropped and as fees, credits, and payments are applied.
Manual Payments
If I elect not to participate in automatic payments, I understand that I remain responsible for making all payments by the applicable due dates. If payment is not received by the due date, I authorize BADA to process any outstanding balance, plus applicable late fees, using the payment method on file.
I specifically authorize Burlington Academy of Dance & Arts to process all required registration payments, including registration fees, August tuition, May tuition, monthly tuition, costume fees, performance fees, summer tuition, workshop fees, camp fees, intensive fees, and any other charges due on my account, using the payment method(s) on file in accordance with the policies outlined above.
This authorization shall remain in effect until revoked in writing and accepted by BADA.
ASSUMPTION OF RISK & RELEASE OF LIABILITY
I understand that participation in dance, acrobatics, fitness activities, rehearsals, performances, classes, camps, intensives, workshops, and related activities involves inherent risks of injury. I voluntarily assume all such risks on behalf of myself and/or my child.
I certify that, to the best of my knowledge, my child is physically capable of participation and that I have disclosed any medical conditions, injuries, allergies, restrictions, or limitations that may affect participation.
I release, waive, discharge, and hold harmless Burlington Academy of Dance & Arts, its owners, directors, officers, employees, faculty members, staff members, volunteers, agents, and representatives from any and all claims, liabilities, demands, damages, costs, expenses, or causes of action arising out of or relating to participation in BADA activities.
EMERGENCY MEDICAL AUTHORIZATION
In the event of illness, accident, injury, or emergency and I cannot be reached, I authorize BADA personnel to obtain emergency medical treatment for my child as deemed necessary. I understand that I am responsible for any resulting medical expenses.
MEDIA RELEASE
I grant permission for BADA to photograph, videotape, record, and use my child's image, likeness, voice, name, and performance in promotional materials, advertisements, publications, social media, websites, video productions, livestreams, and other marketing or educational materials without compensation.
PRIVACY POLICY
BADA respects your privacy and does not sell, rent, or trade personal information. Information collected is used solely for studio operations and administration. Authorized BADA personnel may access information as necessary to conduct studio business or as required by law.
ELECTRONIC SIGNATURE
By selecting "I have read and agree to the terms and conditions," I acknowledge and agree that:
• I have carefully read and understand this Agreement.
• I have had sufficient opportunity to ask questions regarding these policies.
• I agree to be legally bound by its terms.
• My electronic acceptance is the legal equivalent of my handwritten signature.
• I am the parent, legal guardian, or authorized adult responsible for the enrolled student(s).
• I understand that enrollment at Burlington Academy of Dance & Arts constitutes acceptance of all policies outlined above.
