By enrolling in our programs: I acknowledge and understand the following:
Payment Notice:
Electronic payments include a small processing fee (3% for credit cards, 2% for ACH) charged by the payment processor and not by Twinkle Toes Parties, LLC. Online payments are optional—cash or check may be used with no additional fee. Please Note: Registration is not complete until payment is received and spots cannot be held.
1. Program Description: Our summer program offers 2.5-3 hour workshops designed to teach specific skills to children aged 3 to 10 years. These workshops are intended for educational purposes and are not structured as a licensed childcare program.
2. Licensing Information: I acknowledge that the program is not a licensed program nor accredited by the American Camp Association (ACA). Additionally, I understand that the program does not have to abide by the licensing requirements set forth by the state of New Hampshire for childcare programs as our workshops are intended for the sole purpose of teaching a skill.
3. Staff Qualifications: I recognize that our program staff consists of MA & NH certified preschool teachers who are trained to provide a safe and engaging learning environment. We adhere to a 12 to 1 child-to-staff ratio for children in the age group of 5 to 10 years and an 8 to 1 child-to-staff ratio for children in the age group of 3-5 years.
4. Safety and Training: I understand that the staff is up-to-date on necessary training, including First Aid and CPR, and that background checks have been conducted to ensure the safety and well-being of all participants.
5. Waiver of Liability: I voluntarily waive any claims against the Summer Adventure Program (hosted by Twinkle Toes Parties, LLC), its staff, and associated parties for any injuries or incidents that may occur during the workshops. I recognize that participation in the program carries certain risks, and I consent to my child's participation with full awareness of these factors.
6. Understanding of Risks: I have read and understood the nature of the workshops, the lack of licensing and accreditation, and the associated risks. I consent to my child's participation in the program with full awareness of these factors.
7. Emergency Contact: I understand that I am responsible for providing accurate emergency contact information and ensuring that my child is in good health to participate in the workshops.
PROGRAM ACTIVITIES
Here's a list of our summer adventure program activities that include a variety of fun and educational elements: 1. Water Activities: Slip and Slide, Water Balloon Toss, Water Blasters (foam). 2. Play Activities: Outdoor Games (like tag or hide and seek), Scavenger Hunts, Nature Walks. 3. Mud Activities: Mud Puddle Jumping, Mud Painting, Mud Kitchen Play. 4. Kitchen Activities: Cooking Classes (simple recipes), Baking Cookies or Cupcakes, DIY Pizza Making, DIY Snack Making. 5. Sports Activities: Team Sports (soccer, kick ball), Relay Races, Obstacle Course Challenges, Bounce House, FOAM Machine. 6. Playground Activities: Free Play on Playground Equipment, Group Games (like capture the flag), Picnic on the Playground/Pavillion. 7. Arts and Crafts: Nature Crafts (using leaves, stones), Painting and Drawing Sessions, Tie-Dye T-Shirts. 8. Science Experiments: Simple Chemistry Experiments (like volcanoes), DIY Slime Making, Nature Observations and Experiments. 9. Dancing Activities: Dance Parties, Dance Challenges, Creative Movement Sessions, Yoga Sessions, Dance Showcases.
HEALTH REQUIREMENTS
My child is in good health and has no physical or medical limitations that would cause the activities as described above to be detrimental or dangerous to the child. Parents/guardians should specify allergies and medical issues in appropriate section.
ACTIVITY CONSENT
I hereby attest that I am (we are) the legal parent\guardian(s) of the above-named child and hereby consent to the child's participation in the activities described above. I understand that activities of the kind described above may result in physical injury to my child but nonetheless specifically request that he or she be allowed to participate in those activities.
HEALTH INSURANCE
I/We understand that Twinkle Toes Parties, LLC. does not carry any medical insurance relative to the activities or for any injury that may occur to the above-named child. I/We represent that the child is (a) covered by insurance through my own insurance carrier; or (b) that I/We am personally financially responsible for any and all medical costs incurred as a result of the child's injury.
EMERGENCIES
If the above-named child requires any emergency medical treatment or procedures during the activities, I hereby consent to and authorize the named activity supervisor(s) and/or Program Directors to make any decision and take any action to arrange for such procedures or treatments in the discretion of the activity supervisor(s).
RELEASE & IDENTIFICATION
I release and waive, and further agree to indemnify, hold harmless or reimburse Twinkle Toes Parties, LLC., the individual members, agents, Summer Adventure Program, employees and representatives thereof, as well as activity supervisors, from and against, any claim which I, any other parent or guardian, any sibling, the above-named child, or any other person, firm or corporation may have or claim to have, known or unknown, directly or indirectly, for any losses, damages or injuries arising out of, during, or in connection with the child's participation in the activities or the rendering of emergency medical procedures or treatment, if any.
CODE OF CONDUCT
FOR PARENTS:
1. Model Respectful Behavior: Demonstrate respectful communication and behavior towards staff, volunteers, and other families.
2. Support Program Guidelines: Encourage your child to follow the program rules and respect the established guidelines. 3. Engage Positively: Participate in program activities when invited, and promote a positive atmosphere for all. 4. Address Concerns Appropriately: If issues arise, communicate concerns directly with staff in a respectful manner. 5. Encourage Responsibility: Help your child understand the importance of being responsible for their actions and behavior.
FOR CHILDREN:
1. Show Respect: Treat fellow participants and staff with kindness and respect at all times. 2. Follow Directions: Listen carefully to instructions and follow the rules set by staff to ensure a safe environment. 3. Be Inclusive: Include others in activities and be friendly to everyone, fostering a sense of community. 4. Manage Conflicts Calmly: If disagreements occur, resolve them calmly and seek help from staff if needed. 5. Take Responsibility: Own your actions and understand that your behavior affects those around you.
SUNSCREEN/BUG SPRAY/SANITIZER
I give permission for my child to use sunscreen, bug spray, and hand sanitizer during the summer program. I acknowledge the following:
1. Sunscreen Application: I understand that my child may need assistance applying sunscreen throughout the day to protect against sunburn. I will provide sunscreen that is labeled with my child’s name.
2. Bug Spray Application: I consent to the application of insect repellent to my child as needed to prevent bug bites. I will provide bug spray that is suitable for my child’s age and skin sensitivity.
3. Hand Sanitizer Use: I give permission for my child to use hand sanitizer as needed to maintain hygiene, especially before meals and after activities.
4. Health Considerations: I acknowledge that I have informed the program staff of any allergies or sensitivities my child may have to sunscreen, bug spray, or hand sanitizer.
5. Release of Liability: I release the summer program, its staff, and volunteers from any liability related to the application and use of sunscreen, bug spray, and hand sanitizer.
PHOTO/VIDEO RELEASE
I grant permission for Summer Adventure Program (hosted by Twinkle Toes Parties, LLC) to take photographs and videos of my child during program activities. I understand that these images may be used for promotional purposes, including but not limited to brochures, websites, and social media. 1. Usage Rights: I give Summer Adventure Program (hosted by Twinkle Toes Parties, LLC) the right to use my child’s image and likeness in any form of media without compensation. 2. Duration of Use: This permission is granted for an indefinite period unless revoked in writing. 3.Release of Liability: I release Summer Adventure Program (hosted by Twinkle Toes Parties, LLC) from any liability related to the use of these images. SPECIAL SNACKS
I understand that while Summer Adventure Program (hosted by Twinkle Toes Parties, LLC) does not typically provide daily snacks, there may be occasions when special snacks such as popsicles (Popsicle brand), popcorn (Orville Redenbacher brand) and/or fruit (bananas, strawberries, watermelon, blueberries, etc), (Cooking Week may include Cookies, Cupcakes, Pizza) may be offered. I have informed the program staff of any food allergies or dietary restrictions my child may have. Release of Liability: I release Summer Adventure Program (hosted by Twinkle Toes Parties, LLC), its staff, and volunteers from any liability related to the consumption of these special snacks.
