I hereby release and hold harmless DeForest Dance Academy, its owners, members and all employees from all liabilities, suits, claims and/or demands of any kind or nature, legal or financial, arising from the participation in, or observation of, any DeForest Dance Academy activity or event and injuries to any person or property, whether on or off the premises. The participant named above does voluntarily participate in any DeForest Dance Academy activity or event. The participant and parent/guardian understand that there are certain risks with involvement with DeForest Dance Academy and its various formal or informal activities or events. DeForest Dance Academy is not responsible for any lost or stolen property at any time. DeForest Dance Academy has the right to dismiss a participant from a class or event if found to be disruptive to either another individual or group. Anyone found violating the rules and codes of DeForest Dance Academy may be asked to leave the premises or off-site location at any time and be refused to reenter without full or partial refund. Photo/Video Release: I understand that DeForest Dance Academy may record my child’s/my participation in scheduled classes, activities and/or events by photograph or video. I grant DeForest Dance Academy unlimited use of any such recordings with no obligation to me. Such recordings are the sole property of DeForest Dance Academy. Tuition & Fees: Tuition will be billed monthly. A 2% late fee will be assessed on any unpaid balance on the 25th of the month.
Class Make-Up Policy: A maximum of one absence per class per month may be made-up. All make-up classes must be scheduled and completed before June 1st. You may schedule the make-up class by calling or stopping at the front desk. Withdrawal Policy: If your dancer would like to withdraw or change classes, please send a written request to annemarie@deforestdance.com or stop at the front desk and complete a Class Drop/Change Form. Refund Policy: No tuition refund will be given if a dancer withdraws mid-month.
