PARTICIPANT WAIVER AND RELEASE OF LIABILITY
By registering for or attending any class, event, or activity with Mariposa Dance Company, you agree to the following:
Assumption of Risk:
I acknowledge that participation in dance and movement-based activities involves inherent risks, including but not limited to sprains, strains, fractures, and other physical injuries, as well as potential exposure to infectious diseases such as COVID-19 or other viral/bacterial illnesses.
Medical Fitness:
I affirm that I (or my child) am in good physical health and have no known medical conditions that would prevent safe participation in dance classes. It is my responsibility to consult a physician if I have any concerns.
Injury and Illness Liability:
I understand that Mariposa Dance Company, its instructors, staff, and affiliates are not liable for any injury, illness, or loss that may occur during participation in studio activities, whether on-site, off-site, or while arriving/departing the premises.
Emergency Medical Care:
In the event of an emergency, I authorize Mariposa Dance Company staff to seek necessary medical treatment for me or my child. I agree to assume all financial responsibility for any resulting medical expenses.
Missed Classes and Refunds:
I understand that missed classes due to illness, injury, or personal reasons are non-refundable unless explicitly stated by Mariposa Dance Company.
COVID-19 AND COMMUNICABLE DISEASE ACKNOWLEDGEMENT
I understand that, despite the safety precautions taken by Mariposa Dance Company, there is an inherent risk of exposure to COVID-19 or other communicable diseases. By attending in-person classes, I voluntarily assume all such risks and release Mariposa Dance Company from any liability related to exposure or illness.
MEDIA RELEASE
I grant permission for Mariposa Dance Company to use photographs, videos, and recordings of myself or my child for promotional and marketing purposes across print, web, and social media platforms.
PRIVACY POLICY
Mariposa Dance Company is committed to protecting your personal information. We collect and store data such as names, addresses, phone numbers, email addresses, emergency contacts, and medical information solely for studio operations and safety.
Your data will never be:
Sold, rented, or shared with third parties without consent.
Used for unrelated marketing purposes.
Your data may be used to:
Communicate studio news, class updates, and emergency alerts.
Maintain health and attendance records.
Process registrations and payments through secure systems.
Data Security:
All data is securely stored and accessible only to authorized staff. We comply with all applicable privacy and data protection laws.
Parental Consent (if applicable):
I have read, understood, and voluntarily agree to the terms of this waiver, liability release, and privacy policy.
