DANCECAPADE ENROLLMENT AGREEMENT, WAIVER & STUDIO POLICIES

By enrolling myself and/or my child(ren) in Dancecapade programs, I acknowledge that I have read, understand, and agree to the following:

- Assumption of Risk & Release of Liability
I understand that participation in dance, Pilates, tumbling, rehearsals, performances, competitions, private lessons, camps, and related activities involves inherent risks, including but not limited to bodily injury, illness, disability, property damage, or death. On behalf of myself and/or my child(ren), I voluntarily assume all such risks and release, waive, discharge, and hold harmless Dancecapade, Inc., its owners, directors, employees, instructors, contractors, volunteers, and agents from any and all claims, liabilities, damages, or expenses arising from participation in Dancecapade activities, whether on or off studio premises, including claims resulting from ordinary negligence to the fullest extent permitted by California law. I represent that I am the parent or legal guardian of the participating minor and have the authority to enter into this Agreement on the minor's behalf.

- Medical Authorization
I understand that Dancecapade does not provide medical insurance. I authorize Dancecapade staff to administer basic first aid and obtain emergency medical treatment if I cannot be reached. I accept full financial responsibility for all medical expenses incurred.

- Communicable Illness
I acknowledge that participation may expose myself or my child(ren) to communicable illnesses, including but not limited to COVID-19. I agree not to attend classes or studio activities while experiencing symptoms of a contagious illness or following a confirmed diagnosis when participation could expose others.

- Photo & Video Release
I grant Dancecapade permission to photograph or record myself and/or my child(ren) during classes, rehearsals, performances, competitions, and studio events for promotional, educational, and marketing purposes. If I do not wish to be included, I will notify Dancecapade in writing before participation.

- Tuition & Enrollment Policies
I understand that I am financially responsible for all tuition, fees, and charges associated with my account. Monthly tuition is due by the 10th of each month, and the credit card on file may be charged automatically if payment has not been received. Declined payments are subject to a $25 late fee.

To discontinue enrollment in a group class, a Drop or Pause Class Form must be submitted through the Dancecapade website by the 15th of the current month to avoid tuition charges for the following month. Tuition and fees are non-refundable. Tuition credits, if any, are granted solely at Dancecapade's discretion for extended absences due to serious illness or injury supported by medical documentation.

Private lessons require a minimum of 24 hours' notice for cancellation to receive a credit. Late cancellations or missed appointments will be charged the full lesson fee.

- Makeup Classes
Missed group classes may be made up by the end of the month in which the class was missed in an age- and level-appropriate class, subject to availability. A Makeup Class Request Form must be submitted, and proper dance attire and footwear are required.

- Studio Policies
Students are expected to arrive on time and wear the required dress code. Parents are responsible for the timely drop-off and pick-up of their children. Dancecapade is not responsible for students outside of scheduled class times or for lost, stolen, or damaged personal belongings.

Dancecapade reserves the right to modify class schedules, instructors, and programs as necessary.

Dancecapade reserves the right to refuse service, deny enrollment, or dismiss any student or family whose behavior, conduct, or actions are deemed disruptive, unsafe, inappropriate, or not in the best interest of the studio community. Such decisions are made at the sole discretion of Dancecapade. Tuition and fees previously paid are non-refundable unless otherwise determined by Dancecapade.

- Acknowledgment
I certify that I have carefully read this Agreement, understand its contents, and voluntarily agree to all Dancecapade policies, waivers, and releases. I understand that my electronic acceptance of this Agreement has the same legal effect as my handwritten signature and that this Agreement shall be governed by the laws of the State of California.
