Studio Agreement
Parties & Definitions- In this Agreement: “CSPAC” means Cindy Straub’s Performing Arts Academy (also known as Cindy Straub’s Performing Arts Center).
“Registrant” means the individual completing this registration form, and, when enrolling a minor, the Registrant’s parent or legal guardian (“Guardian”).
By clicking “I Agree,” you (the Registrant)—and, if enrolling a minor, the Guardian—acknowledge and accept all terms below on behalf of yourself and any participating minor.
1. Payment Policy
1.1 Autopay Enrollment. Autopay is required for all accounts. Charges will post on the 15th of each month.
1.2 Non-Refundable Fees. Membership, costume, and any other fees labeled “non-refundable” are non-refundable once paid.
1.3 Cancellation & Future Billing. You may cancel at any time by submitting written notice (email). Upon receipt, autopay will cease and you will not be charged for any future months. No refunds will be issued for amounts already paid.
1.4 Returned Payment Fee. A $30 fee applies to any returned payment (paper check or autopay).
1.5 Account Responsibility. You are responsible for maintaining current payment information and ensuring timely funding.
2. Medical Authorization & Emergency Contact
2.1 Emergency Care. In the event of injury or illness during any CSPAC activity, you authorize CSPAC staff to secure medical treatment deemed necessary, including hospitalization, anesthesia, or surgery. You agree to pay all costs.
2.2 Emergency Contact Info. At registration you must provide:

Any relevant medical conditions or allergies (please email CSPAC a copy)
3. Assumption of Risk; Release & Indemnification
3.1 Risk Acknowledgment. Our Classes involves physical activity and risk of injury or damage. You knowingly assume all such risks.
3.2 Release & Indemnification. You, on behalf of yourself and any minor, hereby release, waive, and forever discharge CSPAC, its owners, officers, instructors (including guest/third-party instructors), employees, and affiliates (collectively, “Released Parties”) from any and all claims, liabilities, damages, or causes of action arising from participation. You further agree to indemnify and hold harmless the Released Parties against any claims brought by third parties resulting from your or your minor’s actions.
4. CSPAC Facilities & Third-Party Instructors
4.1 This release covers all CSPAC facilities and equipment (floors, mirrors, seating, décor, or anything else contained on premise) and any instruction or supervision by CSPAC staff or approved third-party/guest instructors.
5. Media Release
5.1 You grant CSPAC and its designees the irrevocable, perpetual right to photograph, film, and record the Participant and to use such media for any promotional, educational, or commercial purposes, in any medium, worldwide, in perpetuity, without compensation.
6. Adherence to Studio Policies
6.1 You agree to follow all posted policies, CSPAC rules, and any health & safety protocols that CSPAC implements from time to time.
7. Severability; Governing Law; Dispute Resolution
7.1 Severability. If any provision is held invalid, the remaining provisions remain in full force.
7.2 Governing Law. This Agreement is governed by the laws of the State of Ohio, regardless of conflict-of-law rules.
7.3 Arbitration. Any dispute arising under this Agreement shall be resolved by binding arbitration in Delaware County, Ohio, under the Commercial Arbitration Rules of the American Arbitration Association. You and CSPAC each waive the right to a jury trial.
8. Acknowledgment & Electronic Signature
By clicking “I Agree,” you confirm that you:
Are at least 18 years old or the parent/legal guardian of any minor participant;
Have read and understand each section of this Agreement;
Have the legal authority to bind yourself and any participating minor; and
Intend your electronic acceptance to be equivalent to a handwritten signature.
