On behalf of my child(ren), I acknowledge that participation in dance activities involves a risk of personal injury. I voluntarily agree to allow my child to participate in activities at Dearborn Dance Academy, and I release Dearborn Dance Academy, its owners, staff, and instructors from any and all claims or liabilities arising from participation, except in cases of gross negligence.

This agreement outlines the expectations and responsibilities of students and parents/guardians enrolled at Dearborn Dance Academy.

By enrolling, you agree to:

Pay tuition by the due date each month.
Follow the dress code for each class.
Arrive on time and prepared for class.
Notify the studio of absences in advance, when possible.
Treat teachers, staff, and peers with respect.

I understand that:

Tuition is non-refundable.
Consistent attendance is crucial for performance readiness.
Costumes and recital fees are separate from tuition.
