Irish Step and Ceili Dance is a challenging aerobic physical activity. Please complete this section to acknowledge and inform the instructors of any limitations for yourself or your dancer.

Any allergies or medical concerns, please inform instructors prior to first class.

LIABILITY:

While all persons acting on behalf of the school do their best to ensure the safety of all dancers, accidents may occur as with any type of physical activity. I agree to not hold Mairead O’Brien, the O’Brien School of Irish Dance, OSIDPA (OBrien School of Irish Dance Parents Association), or any person acting on behalf of the School or Association responsible for any loss, accident, or liability that might occur while my child/children are attending lessons and other Irish Dance related activities. I fully and knowingly agree to assume all such risks associated with dance activities and I further state that my child/children are in good health and have not been restricted from any such activity such as dancing. I have read the dance school policies and fully understand the terms of this release agreement.
