Participation Waiver & Release (Electronic Acceptance)
By creating an account and/or enrolling in classes at Loudoun School of the Arts (“Studio”), I acknowledge and agree to the following on behalf of myself or my minor child:
1. Assumption of Risk – I understand that participation in dance, art, music, and related activities involves risks of injury, and I voluntarily accept those risks.
2. Release of Liability – I release and hold harmless Loudoun School of the Arts, its owners, instructors, staff, and affiliates from any and all claims, damages, or liabilities arising from participation, except in cases of gross negligence or intentional misconduct.
3. Medical Authorization – In the event of an emergency, I authorize the Studio to obtain medical treatment for myself or my child and accept full responsibility for any costs incurred.
4. Photo/Media Release – I grant permission for photos or videos of myself/my child to be used by the Studio for promotional purposes, unless I notify the Studio in writing.
5. Policies & Conduct – I agree to follow all Studio rules, safety guidelines, and policies.
