I, the undersigned parent and/or guardian of the participating minor(s), recognize that the classes offered will involve physical activity. I acknowledge that the above minor is in good health and has neither a disability nor physical impairment that would preclude the minor from participating in the physical activity required for the classes. I also recognize that the classes involving physical activity may result in injury as a result of the activity. Knowing the above for myself and the minor, I do release discharge, acquit and forever forgive Xtreme Tumbling and Dance Center, LLC., it's employees, instructors, agents and officers from any and all present and future actions, causes of actions, civil or criminal claims, demands, damages, costs, attorneys' fees, loss of services, expenses, compensation, third party action, suits at law or in equity, including claims and suits for contribution and/or indemnity or whatever nature, and all consequential damages, and all known and unknown personal injury, death, and/or property damage arising out of participation or taking of any classes or instructions at Xtreme Tumbling and Dance Center, LLC.. I further state that I have carefully read the foregoing release and know the contents thereof and sign the same as my own free act.
