THE COLLAB - STUDENT WAIVER & RELEASE AGREEMENT

1. ASSUMPTION OF RISK
I understand that participation in dance, fitness, and movement-based activities involves physical exertion and carries inherent risks, including but not limited to, slips, falls, sprains, strains, illness (including but not limited to COVID-19), and other physical injuries. I voluntarily choose to participate (or allow my child to participate) in classes and events at The Collab and assume full responsibility for any risk of injury or loss.
2. WAIVER & RELEASE OF LIABILITY
By signing this agreement, I (or my child) hereby release, waive, and discharge The Collab, its owners, instructors, volunteers, agents, and affiliates from any and all liability, claims, demands, or causes of action for injury, damage, illness, or loss arising out of participation in any activities, including use of the facility and equipment, to the fullest extent allowed by law.
3. MEDICAL TREATMENT AUTHORIZATION
In the event of an emergency, I authorize The Collab staff to seek medical treatment deemed necessary for myself or my child. I understand I am responsible for any medical expenses incurred.
4. PHOTO/VIDEO RELEASE
I grant The Collab permission to use photographs and/or video taken during classes or events for promotional purposes, including but not limited to social media, website, and printed materials.

5. CODE OF CONDUCT
The Collab is a faith-based, welcoming space where creativity, worship, and community thrive. I agree to respect the space, instructors, and fellow dancers. Disrespectful, inappropriate, or disruptive behavior may result in removal from class or membership suspension without refund.
6. INDEMNIFICATION
I agree to indemnify, defend, and hold harmless The Collab, its owners, instructors, volunteers, and affiliates from any and all claims, liabilities, damages, or expenses (including attorney's fees) arising from my participation (or my child's participation) in any activity associated with The Collab.
7. MINOR PARTICIPANTS
(If participant is under 18) I certify that I am the parent or legal guardian of the minor listed above and that I have the legal authority to sign this agreement on their behalf.
8. DIGITAL SIGNATURE ACKNOWLEDGEMENT
If this agreement is signed electronically, I understand that my typed name or digital signature is considered legally binding under the U.S. Electronic Signatures in Global and National Commerce (ESIGN) Act.
