Payment Processing
By signing below, I certify that monthly tuition will be collected automatically each month as per the program I selected.
Tuition is collected on the 1st of the month for the month (e.g. On December 1st tuition for the month of December is processed). If the payment is made later than the 5th of the month, $50 late payment fee is charged. If the payment is more than 10 days late (10th of the month), we cannot allow the student on the classes.
By signing this document, I authorize Black & Yellow Dance School to charge my card (or bank account) the sign up fee ($65 for Main Team, $40 for Aerial. Only 1 fee is charged if enrolling into multiple classes). I also authorize Black & Yellow Dance School to charge my card (or bank account) for costume fees and competition registration fees.
All memberships are nonrefundable. Participants in this program are responsible for their own attendance, no credit will be given for missed classes. Additionally, we do not allow accounts to be “held” or “frozen.” In case of injury incompatible with the classes, membership can be stopped immediately. In case of moving away, one month’s notice is required in writing to blackandyellownj@gmail.com . In case of inability to continue classes due to losing a source of income, membership can be stopped immediately.
Acknowledgment of Risk & Waiver of Liability
I understand and agree that in participating in any dance/circus class, workshop, rehearsal or performance, there is a possibility of physical injury. I agree to release and hold harmless Black & Yellow Dance School INC., including its teachers, dancers, staff members, and facilities from any cause of action, claims, or demands now and in the future. I will not hold Black & Yellow Dance School INC. liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. If I am signing this waiver for my child, I certify that I am the parent or legal guardian and have the right to waive these rights.
Furthermore, I understand that I should be aware of my, and/or my child’s physical limitations and agree not to exceed them. I confirm that my child is in good health to participate in this program, and there are no pre-existing injuries, medical conditions, disabilities, or impairments that will prevent my child from participating.
Furthermore, I understand the risks of participating in dance classes and my rights as they pertain to COVID-19, and I understand that I and my child have the option not to participate in Black & Yellow dance classes. By participating in classes, I accept any and all responsibility for transmission of COVID-19 and will not hold Black & Yellow Dance School INC., including its teachers, dancers, staff members, and facilities, liable for transmission of COVID-19.
Photo & Video Release
Further, I authorize and agree that Black & Yellow Dance School INC. may take and use photographs/videos of me and/or my child for purposes of record keeping, advertising, and marketing.

No Class Days
We observe national holidays and school off days.
Weather/Unscheduled Closure
If we are forced to close due to inclement weather or extreme emergency, we will announce it in our WhatsApp group chat. Please understand that we cannot be responsible for acts of Mother Nature. Snow dates do not affect your monthly tuition.
Drop-off only
Please drop-off only. No parents inside the studio during the time of the classes. We want to make sure the students are focused on the class and the only adults in the studio at the time of the class are coaches. We are posting pictures/videos from classes to group chats and we have 3 performances during the year that parents and friends are welcome to attend.
Late pick-up
Please drop off and pick up kids on time. Late pick-up may be subject to a fee $15-$100.
Groups size/Coaches
Number of kids in the group is subject to change. We have a strict limit on the maximum amount of kids in each group. Coaches are subject to change.
Cellphone/Toys Policy
Students are not allowed to have cell phones or toys in the classroom as it is disruptive to the class. All cell phones and toys must be left in the Waiting Room. The studio is not responsible for any lost or misplaced items.
Student Storage & Lost Items
Studio provides free storage space for active students. Only yoga mats, yoga blocks and shoes are allowed. All yoga mats must be rolled and signed. All yoga blocks must be signed. Students should do their best to keep the Storage Room organized. The studio is not responsible for any lost or misplaced items.
Bullying/Harassment Policy
Our studio is a place where everyone should feel safe, accepted, and have a sense of belonging. Bullying/Harassment will never be tolerated here. Black & Yellow Dance and Circus School believes in creating a caring and supportive environment in which students are encouraged and challenged to develop their strengths, passions, and talents.

Dress Code
Sport active wear. Yoga mat and yoga blocks for Contortion class. Hair – Must be pulled back off of the face in either a bun or ponytail. Clean indoor sneakers for Dance class. Red hair tie on right wrist and right ankle for beginner Contortion group.
NO jeans, beach shorts, jewelry, dresses, skirts, belts, loose clothing, outdoor shoes, items in pockets, clothing with other dance studio's logos.
