I authorize Ozark LegacyDance Academy LLC, staff to seek emergency medical treatment for myself or my child if necessary. I understand that every effort will be made to contact me in the event of an emergency.

Ozark Legacy Dance Academy LLC.
1. Tuition
Tuition is posted at registration and by the last day of the previous month to be paid on a 1st of the month basis and is due by the [1st of each month/first class of the session]. Tuition is non-refundable and is based on enrollment, not attendance.
2. Payment Methods
We accept payments by [credit card, debit card, ACH bank transfer, cash]. Accounts will be set on auto pay. Payments can be made online in advance through our parent portal or via check before the 1st of each month.
3. Late Fees
A late fee of $15 will be applied to any tuition not received by the 10th of the month tuition is due.
4. Missed Classes & Make-Ups
Missed classes are non-refundable. Makeup classes may be offered based on availability—please contact us in advance to schedule.
5. Withdrawal
If you choose to withdraw from a class, [written/email] notice is required at least 30 days before your next billing cycle. You are responsible for any tuition due until the withdrawal notice is received.
6. Returned Payments
Returned checks or declined credit card payments will result in a $25 fee.
7. Additional Fees
Additional fees may apply for costumes, recital participation, workshops, and other special events. These will be communicated in advance.

I grant permission for photos or videos of myself or my child to be used for promotional purposes (social media, website, print materials) for Ozark Legacy Dance Academy LLC.

I agree to abide by all studio policies, including dress code, payment policies(see section), and behavior expectations, which are provided by Ozark Legacy Dance Academy LLC and set forth in the studio handbook.
Dress Code for Dancers at Ozark Legacy Dance Academy LLC.
To help dancers feel prepared, comfortable, and safe, we ask that all students follow our dress code for each class style:
Ballet
• Leotard and tights
• Ballet bun (hair secured neatly away from face)
• Ballet shoes (appropriate color/style per class level)
Acro, Jazz, Contemporary, Tap, Leaps & Turns
• Leotard with shorts or leotard with leggings
• Appropriate footwear:
• Acro: bare feet
• Jazz/Contemporary/Leaps & Turns: jazz shoes or half soles
• Tap: black tap shoes style depends on age
Hip Hop
• Sweatpants or athletic pants (no jeans)
• T-shirt or fitted athletic top
• Clean, white non-street shoes (reserved for studio use only—no outdoor shoes allowed in the studio)
No Bra Tops or Crop Tops in Class
Behavior Expectations for Dancers at Ozark Legacy Dance Academy LLC.
To create a positive and safe learning environment for all, we ask that every dancer follows these expectations while participating in class:
1. Respect
Treat instructors, classmates, staff, and studio property with kindness and respect at all times.
2. Listening & Participation
Follow instructions from teachers and participate actively and positively in class activities.
3. Safety First
Keep hands and feet to yourself. No running, pushing, or roughhousing in the studio or common areas.
4. Positive Attitude
Come to class with a positive attitude, ready to learn and support fellow dancers.
5. Be Prepared
Arrive on time, dressed appropriately for class, and with hair secured. Bring necessary dance shoes and a water bottle.
6. No Disruptions
Cell phones should be silenced and put away during class. Food, gum, and drinks (other than water) are not allowed in the dance rooms.
7. Bullying & Harassment
Bullying, teasing, or harassment of any kind will not be tolerated at Ozark Legacy Dance Academy LLC. We strive to create a welcoming environment for every dancer.
