TUITION POLICY: Tuition is DUE, billed, and automatically processed on the 1st of the month. A ten-day grace period is given at which time a $15 late fee will be applied if tuition has not been received. After 30 days, a 10% fee will be added to the TOTAL amount owed. After 60 days of non-payment, the student will not be permitted to attend class until the account has been brought current. If an account goes past due more than 3 consecutive times, students will be put on hold and a meeting with the Director, Karyn Laver, will be required prior to resuming classes.

CLASS CARD POLICY: Class cards are valid for 365 days from date of purchase and can be used for any classes on the schedule of the same class length (45-60 mins or 75-90 mins). Once a class card has been fully used, students will be automatically charged for a class card of the same type unless the account has been frozen or terminated, or switched to monthly tuition.

OTHER PAYMENT POLICIES: Bounced checks will incur a $35 fee. An annual registration fee of $25 will be charged on the first day of September each year. FREEZE POLICY: Accounts can only be frozen for a maximum of two months, and freeze forms must be submitted by the 25th of the month prior to the month you wish to freeze. Freeze fees are as follows: classes below capacity are $15 per enrolled class and classes at full capacity are full tuition price. TERMINATION POLICY: Class withdrawals do NOT take effect in the current month. The withdrawal survey must be completed before the 25th of the current month for the withdrawal to take effect for the following month. Your class will NOT be dropped or changed by verbally informing your instructor, the front office staff, or the director. You MUST complete the withdrawal form for your change to take effect. NO REFUNDS will be given for classes not attended. REMEMBER, if you need to withdraw from classes but do not complete a withdrawal form, your tuition will continue to be charged and you will be responsible for tuition until the time we receive your completed withdrawal form. RELEASE & WAIVER OF LIABILITY & INDEMNITY AGREEMENT: In consideration of being permitted to utilize the facilities, services and programs of The Dance Network (or for myself or my children to so participate) for any purpose, including, but not limited to observation or use of facilities or equipment, or participation in any off-site program affiliated with The Dance Network, I, for myself and/or my participating child(ren) and any personal representatives, heirs, and next of kin, hereby acknowledge, agree and represent that I, or immediately upon entering or participating, will inspect and carefully consider such premises and facilities or the affiliated program. It is further warranted that such entry into The Dance Network for observation or use of any facilities or equipment or participation in such affiliated program constitutes an acknowledgment that such premises and all facilities and equipment thereon and such affiliated program have been inspected and carefully considered and that I find and accept the same as being safe and reasonably suited for the purpose of such observation, use or participation by myself and/or my child(ren). In further consideration of being permitted to enter The Dance Network for any purpose including, but not limited to, observation or use of facilities or equipment, or participation in any off-site program affiliated with The Dance Network, I hereby agree to the following: 1. I, on my behalf and/or the behalf of my children, hereby release, waive, discharge and covenant not to sue The Dance Network, its directors, officers, employees, and agents (hereinafter referred to as "releases") from all liability to myself or my child(ren) and all my personal representatives, assigns, heirs, and next of kin for any loss or damage, and any claim or demands therefore on account of injury to the person or property or resulting in death of myself and/or my child(ren), whether caused by the negligence of the releases or otherwise while I or my child(ren) is in, upon, or about the premises or any facilities or equipment therein or participating in any program affiliated with The Dance Network. 2. I hereby agree to indemnify and save and hold harmless the releases and each of them from any loss, liability, damage or cost they may incur due to the presence of myself or my child(ren) in, upon or about The Dance Network premises or in any way observing or using any facilities or equipment of the The Dance Network or participating in any program affiliated with The Dance Network whether caused by the negligence of the releases or otherwise. 3. I hereby assume full responsibility for and risk of bodily injury, death, or property damage to myself and/or my child(ren) due to negligence of releases or otherwise while in, about or upon the premises of The Dance Network and/or while using the premises or any facilities or equipment thereon or participating in any program affiliated with The Dance Network. I further expressly agree that the foregoing release, waiver, and indemnity agreement is intended to be as broad and inclusive as is permitted by the law of the State of California and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. I have read and voluntarily, electronically sign the release and waiver of liability and indemnity agreement, and further agree that no oral representations, statements, or inducement apart from the foregoing written agreement have been made.

MEDICAL EMERGENCIES: In the event of physical injury or medical emergency, I hereby agree and authorize The Dance Network to make necessary arrangements to transport myself and/or my child(ren) to a medical treatment facility as necessary. All such transportation and medical treatment will be at my sole cost and expense. In extreme emergencies, or if participating children are under 18 years of age, I understand that The Dance Network will attempt to notify the person(s) named as emergency contact(s) of the situation and how to reach me.

PHOTO RELEASE: The Dance Network reserves the right to use photographs and videos taken during classes, workshops, performances, or other affiliated events for the purposes of instruction, advertising, and promoting The Dance Network and its programs. I agree to allow my image and likeness or that of my child(ren) to be used by The Dance Network for instructional and marketing purposes. I understand that I will not be compensated for the use of my image or likeness. If I would prefer for myself and/or my child(ren) not to appear in any photos or videos used by The Dance Network, it is my duty to proactively communicate this to The Dance Network prior to participation in any class.
