We require you to take part in our auto draft program for monthly tuition classes which deducts from your bank account on the 3rd of each month. This does not apply to adult punch cards. You will receive monthly invoices, we will deduct the amount of the invoice. In October, we will bill for a one time recital fee and costume fees.

Class Assignments are made by teachers. If you are unsure about assignments for your child please call the studio. We are happy to discuss classes and levels.

Release of Liability
As the legal parent or guardian, I release and hold harmless the Columbia Ballet School, the owner of the premises where the dance class education classes take place, or auxiliary staff from liability in connection with said dance education classes.

Health and Insurance
I declare that I or my child is in good health and can participate in physical activities, namely dance and exercise classes. My physician, named on this form, is aware of my or my child participation in the activities and has given his or her permission to participate. Given the nature of the physical education classes, and with the knowledge that injuries might occur, I have taken the necessary steps to obtain accident, health or hospitalization insurance to cover any sustained injury.

Payment Information
Registration and last month's tuition due when you register. There will no be any refunds for either and no refunds given for missed classes.

Photographs may be taken during classes, rehearsals, and performances. These photographs are used on the school's website and in promotional materials. Names of children are not used without parental permission. I agree that my student's photograph may be used for this purpose.

The registration fee is $90 to complete your registration for Columbia Ballet School. Classes are filled on a first-come, first-served basis. When you submit this registration on-line, you will receive an email confirmation followed by your first invoice, which is due immediately. You may also call for help with your registration or come by to the studio office at 132 Gills Creek Parkway, Columbia, 29209.
