The Edge Performing Arts – Liability Waiver and Photo/Video Release

Liability Waiver
By signing this form, I acknowledge and agree to the following:
I understand that participation in dance classes, rehearsals, performances, and related activities at The Edge Performing Arts (located in Roseville, CA) involves physical activity that carries inherent risks, including but not limited to: strains, sprains, falls, and other potential injuries.
I voluntarily assume all risks associated with my/my child’s participation in any activities at The Edge Performing Arts. I hereby release, waive, and discharge The Edge Performing Arts, its owners, directors, instructors, employees, volunteers, and affiliates from any and all liability, claims, demands, causes of action, or damages arising out of or related to any injury, illness, accident, or loss that may occur while participating in classes, events, or while on the premises.
I certify that the participant is physically able to take part in dance activities and that I will notify staff of any health conditions or limitations that may affect participation.

Photo/Video Release
I grant permission to The Edge Performing Arts to photograph and/or record video footage of my child during classes, events, or performances. I understand that these images or videos may be used for promotional purposes, including but not limited to: social media, website, marketing materials, or press releases.
I understand that no compensation will be provided for the use of such media and that all images and videos will be the property of The Edge Performing Arts.
