Liability Waiver, Medical Authorization & Photo Release
Assumption of Risk
I voluntarily enroll myself and/or my child in classes, rehearsals, performances, camps, workshops, conditioning programs, competitions, and other activities offered by The Forge Dance Company ("the Studio").
I understand that participation in dance, tumbling, conditioning, fitness activities, and related programs involves inherent risks. These risks include, but are not limited to, slips, trips, falls, sprains, strains, fractures, muscle soreness, overuse injuries, contact with other participants or equipment, and other injuries that may occur during Studio activities.
While The Forge Dance Company is committed to providing a safe and supportive learning environment through appropriate instruction and supervision, I understand that no physical activity is entirely risk-free.
Health & Participation
I certify that I (or my child) am physically able to participate in Studio activities. I agree to inform the Studio of any medical conditions, injuries, allergies, or physical limitations that may affect safe participation.
If I or my child becomes ill or injured, I understand that participation may be limited or suspended until it is safe to return.
Medical Authorization
In the event of an illness or injury requiring emergency medical care, I authorize The Forge Dance Company staff to obtain appropriate emergency medical treatment for myself or my child if I cannot be reached.
I understand that I am responsible for any medical expenses incurred.
Release of Liability
To the fullest extent permitted by law, I voluntarily release, waive, and hold harmless The Forge Dance Company, its owners, instructors, employees, independent contractors, volunteers, and representatives from any claims, liabilities, damages, costs, or expenses arising from participation in Studio activities, except to the extent caused by gross negligence or willful misconduct.
This release applies to participation in classes, rehearsals, private lessons, performances, competitions, camps, workshops, conditioning classes, Studio events, and reasonable travel associated with Studio activities.
Insurance
I understand that The Forge Dance Company does not provide medical or accident insurance for participants. Maintaining personal health insurance is the responsibility of each participant or parent/guardian.
Photo & Video Release
I grant permission for The Forge Dance Company to photograph and/or record myself or my child during Studio activities.
I understand that these images and videos may be used for Studio purposes, including the Studio website, social media, advertising, promotional materials, printed publications, and displays. No personal identifying information will be published without additional permission unless required by law.
Acknowledgment
By accepting this agreement, I acknowledge that I have carefully read and understand this Liability Waiver, Medical Authorization, and Photo Release. I understand the risks associated with participation in Studio activities, voluntarily accept those risks, and agree to the terms stated above. If I am accepting this agreement on behalf of a minor participant, I certify that I am the participant's parent or legal guardian and have the authority to accept this agreement.
