I authorize the Studio ACE to photograph and/or video record me or my child during classes, rehearsals, and events, and to use these images or recordings for promotional purposes, including social media, the website, advertising, and marketing materials.

I understand that Studio ACE collects personal information, including names, contact details, emergency contacts, and payment information, for registration, communication, and administrative purposes. This information will be kept confidential and used only for studio operations, safety, and communication. The studio will not sell or share personal information with third parties except as required by law or as needed to process payments or provide services. I understand that I may request access to, correction of, or deletion of my information at any time by contacting the studio.

I understand that participation in dance classes, rehearsals, performances, and related activities involves physical activity and carries a risk of injury. I voluntarily assume all risks associated with participation for myself or my child.

In consideration of being allowed to participate, I hereby release, waive, discharge, and hold harmless the Studio ACE, its owners, instructors, staff, contractors, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, loss, or damage that may occur while participating, whether caused by negligence or otherwise, to the fullest extent permitted by Texas law.

I certify that I (or my child) am physically able to participate and have no medical conditions that would prevent safe participation. I agree to inform the studio of any relevant medical conditions.

I have read and fully understand this agreement and agree voluntarily.

Tuition Schedule: I understand that tuition is due on the 1st of each month for all recreational and competitive classes. Tuition amounts for each class type will be communicated at the time of registration and any changes will be provided in writing.

Late Payment Policy: I agree that a late fee of $25 will be applied to any tuition payment not received within five (5) days of the due date. Late fees will continue to accrue in $25 increments for each additional 5-day period past the due date until the balance is paid in full.

Recreational and Competitive Classes: I understand that tuition covers scheduled class time for recreational and competitive dancers, and that competitive dancers may also incur additional fees for choreography, competitions, costumes, or other program-related costs, which will be communicated separately in advance.

Payment Responsibility: I acknowledge that I am responsible for ensuring all tuition and associated fees are paid on time. Failure to make timely payments may result in suspension from classes, rehearsals, or competitions until the account is current.

Acknowledgment: By signing this agreement, I confirm that I understand and accept the tuition schedule, payment policies, and late fee structure for all dance classes in which my child is enrolled.
