1. Assumption of Risk & Release of Liability
I understand that participation in dance classes and related activities (including rehearsals, performances, and events) at APC Dance involves physical movement and carries a risk of injury. I agree to assume all responsibility for any personal injury, illness, or damage to personal property resulting from participation. I hereby release, waive, and hold harmless [APC Dance, its owners, instructors, staff, and affiliates from any and all liability for injuries or damages sustained by the participant while attending or participating in any studio-related activity.
2. Medical Emergencies
In the event of an emergency, I authorize APC Dance and its staff to secure appropriate medical care for the participant. I understand that I am responsible for any medical expenses incurred as a result of injury or illness.
3. Media Release
I give permission for APC Dance to use photographs and/or videos of the participant for promotional purposes, including but not limited to social media, website, marketing materials, and local publications.
4. Studio Policies Acknowledgment
I have read and agree to abide by the studio’s policies, including but not limited to attendance, dress code, payment schedules, and behavior expectations. I understand that failure to comply may result in dismissal from the program.
