I agree to receive messages about my Childs dance tuition and dance information for the upcoming year.

I hereby release the Madison-Mayodan Recreation Department and staff from any and all liability, claims, or demands on account of or related in any way to any injury or disease that my child may suffer as a result of the forgoing and assume all risk in connection therewith and I forte stat that my consent in indeed informed.
I also give my child permission to participate in the Madison-Mayodan Recreation Department's dance program with the understanding that in case of an emergency, I hereby give my permission to the physician selected buy the recreation staff to secure proper treatment for any illness or accident that may happen to my child. This hereby relieves the Madison-Mayodan Recreation Department and staff.
