ST. PETE IRISH DANCE LLC - WAIVER OF LIABILITY AND RELEASE FORM

In consideration of being allowed to participate in Irish dancing classes (the “Activity”) offered by St. Pete Irish Dance LLC (“the School”), I, the undersigned participant (or parent/guardian if under 18), agree to the following:

1. Assumption of Risk

I understand that participation in Irish dancing involves physical activity that carries a risk of injury, including but not limited to slips, falls, sprains, strains, or more serious injuries. voluntarily assume all risks associated with my (or my child’s) participation in classes, rehearsals, performances, or related events.

2. Release and Waiver of Liability

I hereby release, waive, discharge, and hold harmless St. Pete Irish Dance LLC, its owners, instructors, employees, contractors, volunteers, landlords, and affiliates from any and all liability, claims, demands, actions, or causes of action related to any injury, illness, or loss that may occur during participation in the Activity, including travel to and from the Activity.

3. Medical Authorization

I certify that I (or my child) am in good physical health and able to safely participate in Irish dance. In the event of an emergency, I authorize St. Pete Irish Dance LLC staff to seek medical care for me (or my child), and I accept full responsibility for any resulting medical expenses.

4. Photo/Video Release

I grant permission for St. Pete Irish Dance LLC to take photographs and video footage of me (or my child) during classes, events, or performances. I understand that no names will be used without additional consent.

5. Minor Participants

If the participant is under 18 years of age, this waiver must be signed by a parent or legal guardian. The parent/guardian agrees to be bound by the terms of this agreement and accepts full responsibility for the minor’s participation.

6. Governing Law

This waiver shall be governed by the laws of the State of Florida. Any legal disputes shall be resolved in a court of competent jurisdiction located in or nearest to St. Petersburg, Florida.
