You must be over 18 to register for an account.

I acknowledge that I will receive a copy of the Celestial Stars Studio Handbook upon registration, and I agree to follow all policies and procedures outlined within including dress code, behavior expectations, attendance, and communication guidelines. I understand I may request a copy in advance if desired.

I understand that tuition is non-refundable and due on the 1st of each month. Late fees apply after the 5th. I agree to automatic billing and understand that additional fees apply for costumes and performances.

All tuition is processed by automatic withdrawal, which is required for monthly tuition plans.

Processing fees for payment transactions are as follows:
Credit Card: 3.05% + $0.30 per transaction
Bank Withdrawal (ACH): 1% + $0.20 per transaction
These are charged by our processing vendor and will be automatically added to each transaction.

I understand that tuition caps for single students and discounts must be manually applied by the studio, and I agree to contact the studio for any tuition adjustments.

I understand that if my dancer withdraws, written notice must be given by the 15th of the month. No withdrawals are accepted after Feb. 1.

I understand that participation in the annual Revue is mandatory, and that attendance and rehearsal policies must be followed to ensure performance eligibility.

I agree to discuss and receive approval for any outside dance teams, performances, or training that may conflict with Celestial Stars programming.

I give permission for Celestial Stars to use photos and videos of my dancer for promotional purposes. Names will not be used without permission.

I release Celestial Stars and its staff from liability in the event of injury and have disclosed any relevant medical concerns.

I agree to uphold the studio’s social media policy, which includes respectful, positive online conduct and restrictions on sharing class or performance videos.

I understand that bullying, gossip, or disrespectful behavior may result in dismissal from the studio.

I agree to direct all studio-related communication through the Celestial Stars administrative office and not contact instructors personally, unless otherwise specified. Parents should communicate with the Studio Administrative Assistant as the first point of contact.
