Once you register a student, whether online or in person, you agree to release and discharge the Center Dance Academy and staff from any and all actions, causes of action, for damage, loss or injury, which may be sustained by him/ her of his/ her parent/ guardian in consequence of his/ her membership in the school. He/ She also agrees that the Center for the Performing Arts shall not be held responsible for any loss or theft of his/ her personal possessions.

Tuition & Fees
Tuition is due on the 1st of each month. Late payments will result in a $[fee] late charge.
Costume payments for performances must be made by the posted deadlines.

Studio Rooms
We have created a list of what is not permitted in the individual studio rooms during classes. Please follow all rules!
1. No street shoes
2. No food
3. No parent/guardians are permitted in the studio without permission from teacher.
4. No students are permitted to practice in empty studio without supervision of a staff member.
5. No parents/guardians are permitted to watch dress rehearsals.

Waiver
Once you register a student, both online or in person, you agree to release and discharge the Center for the Performing Arts and teachers from any and all actions, causes of action, for damage, loss or injury, which may be sustained by him/ her of his/ her parent/ guardian in consequence of his/ her membership in the school. He/ She also agrees that the Center for the Performing Arts shall not be held responsible for any loss or theft of his/ her personal possessions.

I, the undersigned, hereby grant permission to The Center Dance Academy, its representatives, and employees the right to take photographs and video footage of myself and/or my child during classes, rehearsals, performances, and studio events.

I authorize The Center Dance Academy to use such photographs and video footage, with or without my/our name(s), for any lawful purpose, including but not limited to:

Social media platforms (Instagram, Facebook, TikTok, etc.)

Studio website

Printed promotional materials

Digital marketing and advertisements

Newsletters and press releases

I understand that these images and videos may be used to promote the studio's programs and community, and that no compensation will be provided for the use of these materials.
