I hereby agree to release Dance Now Dance Studios, LLC and hold Dance Now Dance Studios, LLC harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dance on behalf of the participant.

I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of dance. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/ or causes of action of any kind arising as a result of such activity from which liability could accrue to Dance Now Dance Studios, LLC, it’ officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities (hereinafter collectively referred to as “Dance Now Dance Studios, LLC.

I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.


The participant has my permission to participate in Dance Now Dance Studios, LLC Events.

Media Release
I give Dance Now Dance Studios, LLC permission to use my child’s picture in or on any form of advertisement for Dance Now Dance Studios, LLC or a Dance Now Dance Studios, LLC affiliated event.
I, the undersigned, give permission for Dance Now! Studios to use video footage and / or photographs of my child/ward. This usage may include (but is not exclusive to) displaying publicly, distributing, or publishing, photographs, and/or video of my child for use in materials that include, but may not be limited to:
- printed materials (eg - brochures and newsletters)
- online and offline advertising and promotion
- videos and digital images such for use on Social Media.

By signing this form, I acknowledge that I am giving unrestricted permission for my child’s image to be used in print, video, and digital media. I agree that these images may be used by Dance Now! Studios for a variety of purposes and that these images may be used without further notification. I do understand that any identifying information including surname and location will not be used in conjunction with any video or digital images.

Tuition for the full season is broken into 9 equal payments, September through May.

Tuition is due monthly by the first of every month. If payment is not received by the 10th of the month, a $15.00 Late Fee will be added to your account. Payments are preferred to be made through your Portal Account. Payments can be made in person at the studio front desk as well. If your account is more than 30 days past due, your dancer will not be able to dance until it is paid in full.

Pre paid tuition is non-refundable. In the event you discontinue a class, a written notice is required 30 days in advance. Tuition payments will continue to be your responsibility until a 30-day advance notice is received. No credit for missed or dropped classes.

If you discontinue a class after December 20th, all remaining costume balances are your responsibility

Registration Fee: $25.00
Annual Recital Fee: $50.00, includes 1 digital copy of recital, additional rehearsal time and recital time. Due December 30th.
Recital Costume Fee: $85.00-$115.00. Due December 20th. Includes costume, recital tights, accessories
Each class will receive one costume. Combo classes will receive a two in one costume for similar cost.

ASSUMPTION OF RISK/WAIVER OF LIABILITY/INDEMNIFICATION AGREEMENT
In consideration of being allowed to participate on behalf of Dance Now Dance Studios, LLC dance program and related events and activities, the undersigned acknowledges, appreciates, and agrees that:
1. Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenzas, and COVID-19. While rules and personal discipline may reduce this risk, the risk of serious illness and death does exist and,
2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, and assume full responsibility for my participation; and,
3. I willingly agree to comply with the stated and customary terms and conditions for participation regarding protection against infectious diseases. If, however, I observe any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Dance Now Dance Studios LLC, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, and owners used to conduct the event (“RELEASSES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OR RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
